About Koselugo
Overview
Koselugo capsules contain selumetinib, a kinase inhibitor. It is used to treat adults and children aged 1 year and older with neurofibromatosis type 1 (NF1) who have plexiform neurofibromas that cause symptoms and cannot be removed by surgery. Selumetinib blocks proteins called MEK1 and MEK2, which are part of a pathway involved in cell growth.
What it treats
Neurofibromatosis type 1 (NF1) with plexiform neurofibromas that cause symptoms and cannot be removed surgically, in adults and children aged 1 year and older.
Safety information
Side effects
Common side effects in children include vomiting, diarrhea, increased blood levels of creatine phosphokinase (CPK), dry skin, inflammation around the nails, nausea, an acne-like rash and fever. In adults, common side effects include rash, including an acne-like rash, and diarrhea. Serious side effects include reduced heart pumping function, eye problems that can affect vision, severe diarrhea, severe skin rashes, increased CPK levels and muscle breakdown (rhabdomyolysis). Contact your doctor promptly for new or worsening vision changes, severe skin rash or severe diarrhea. For diarrhea, the label advises starting an anti-diarrheal medicine after the first loose stool and increasing fluid intake. Heart checks, eye examinations and CPK blood tests are needed before and during treatment. Serious side effects may require your prescriber to interrupt or stop treatment. Koselugo capsules contain vitamin E. Vitamin E intake above recommended or safe limits may increase bleeding risk. Bleeding risk may also be increased when Koselugo is taken with vitamin K antagonists or antiplatelet medicines. Contact your doctor if bleeding occurs. Koselugo can harm an unborn baby. Effective contraception is needed for patients who could become pregnant or father a child. Contact your doctor if pregnancy occurs during treatment.

Before taking this medicine
The FDA label lists no contraindications for Koselugo. However, treatment requires caution and monitoring for reduced heart pumping function, eye problems, diarrhea, severe skin rashes, and raised creatine phosphokinase (CPK), which may indicate muscle damage. Serious muscle breakdown (rhabdomyolysis) can occur. Heart function, eye examinations and CPK blood tests are needed before and during treatment. Koselugo capsules contain vitamin E. Excess vitamin E intake may increase bleeding risk, particularly when taken with vitamin K antagonists or antiplatelet medicines. Tell your prescriber about these medicines and any vitamin E supplements. Koselugo can harm an unborn baby. People who could become pregnant should use effective contraception. Do not breastfeed during treatment.
Interactions
Tell your doctor about all medicines you take. Koselugo should not be taken with fluconazole or medicines that strongly or moderately block or increase the activity of the enzyme CYP3A4. Your doctor can check whether any of your medicines fall into these groups. If fluconazole or a CYP3A4-blocking medicine cannot be avoided, your prescriber will need to reduce your Koselugo dose.

Dosing
Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.
Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
