About Kisqali

Overview

Kisqali (ribociclib) is a prescription medicine supplied as tablets. It is used with an aromatase inhibitor as additional treatment for adults with hormone receptor (HR)-positive, HER2-negative stage II or III early breast cancer at high risk of returning. It is also used for adults with HR-positive, HER2-negative advanced or metastatic breast cancer, with an aromatase inhibitor as initial hormone-based treatment, or with fulvestrant as initial hormone-based treatment or after the cancer has progressed on hormone therapy. It works by blocking proteins called CDK4 and CDK6 that help cells grow and divide.

What it treats

- HR-positive, HER2-negative stage II or III early breast cancer at high risk of recurrence in adults, as adjuvant treatment with an aromatase inhibitor. - HR-positive, HER2-negative advanced or metastatic breast cancer in adults, with an aromatase inhibitor as initial endocrine-based therapy, or with fulvestrant as initial endocrine-based therapy or after disease progression on endocrine therapy.

Safety information

Side effects

Common side effects include infections, headache, nausea and tiredness. Blood tests may show reduced white blood cells, hemoglobin and platelets, raised liver enzymes and increased creatinine. In people with advanced or metastatic breast cancer, common side effects also include diarrhea, vomiting, constipation, hair loss, cough, rash, back pain and low blood sugar. Serious side effects that warrant contacting a doctor include: • Lung inflammation, which can be severe, life-threatening or fatal. Contact a doctor promptly for new or worsening breathing symptoms. • Severe or life-threatening skin reactions, including Stevens–Johnson syndrome, toxic epidermal necrolysis and DRESS. Contact a doctor promptly if a skin reaction develops. • QT interval prolongation, a change in the heart’s electrical activity. • Liver injury. • Neutropenia, a reduction in a type of white blood cell. Some serious effects are detected through monitoring rather than symptoms. ECGs and blood tests are needed before and during treatment to check the heart’s electrical activity, electrolytes, liver function and blood cell counts.

- Seek medical attention immediately for new or worsening breathing problems or cough. - Seek medical attention immediately for a severe skin rash.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no formal contraindications for Kisqali. However, important warnings and precautions apply: • Kisqali can cause severe, life-threatening or fatal lung inflammation. New or worsening breathing symptoms need prompt medical assessment. • Severe skin reactions, including Stevens-Johnson syndrome, toxic epidermal necrolysis and DRESS, can occur. • Kisqali can prolong the QT interval, affecting the heart’s electrical rhythm. ECGs and blood electrolyte tests are needed before and during treatment. Avoid use with medicines known to prolong the QT interval or strong CYP3A inhibitors. • Kisqali is not indicated for use with tamoxifen because of an increased risk of QT prolongation. • Liver toxicity and neutropenia (low levels of a type of white blood cell) can occur. Liver function tests and complete blood counts are needed before and during treatment. • Kisqali can harm an unborn baby. Patients who could become pregnant should use effective contraception during treatment. • Do not breastfeed during treatment.

Interactions

Kisqali should not be taken with strong CYP3A inhibitors or strong CYP3A inducers. If a strong CYP3A inhibitor cannot be avoided, your prescriber will need to adjust your Kisqali treatment. Kisqali can also affect medicines processed by CYP3A, and your prescriber may need to adjust those medicines. Avoid taking Kisqali with medicines known to prolong the QT interval, an electrical timing measure in the heart. These include certain medicines used to treat irregular heart rhythms. Discuss any such medicines with your doctor before taking them with Kisqali.

Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
