Postmarketing cases of metformin-associated lactic acidosis have resulted in death, hypothermia, hypotension, and resistant bradyarrhythmias. The onset of metformin-associated lactic acidosis is often subtle, accompanied only by nonspecific symptoms such as malaise, myalgias, respiratory distress, somnolence, and abdominal pain. Metformin-associated lactic acidosis was characterized by elevated blood lactate levels (>5 mmol/Liter), anion gap acidosis (without evidence of ketonuria or ketonemia), an increased lactate/pyruvate ratio; and metformin plasma levels generally >5 mcg/mL.
Risk factors for metformin-associated lactic acidosis include renal impairment, concomitant use of certain drugs (e.g., carbonic anhydrase inhibitors such as topiramate), age 65 years old or greater, having a radiological study with contrast, surgery and other procedures, hypoxic states (e.g., acute congestive heart failure), excessive alcohol intake, hepatic impairment, and mitochondrial diseases.
Steps to reduce the risk of and manage metformin-associated lactic acidosis in these high risk groups are provided in the full prescribing information.
If metformin-associated lactic acidosis is suspected, immediately discontinue JENTADUETO XR and institute general supportive measures in a hospital setting. Prompt hemodialysis is recommended.

Overview
This medicine combines linagliptin and metformin hydrochloride to help control blood sugar in adults with type 2 diabetes. It is used alongside diet and exercise and is not recommended for type 1 diabetes. Linagliptin helps increase insulin release in response to blood sugar, while metformin reduces sugar production in the liver and helps the body use insulin more effectively.

Safety information
Side effects
Common side effects include nose and throat inflammation (cold-like symptoms) and diarrhea. Serious side effects listed in the label include lactic acidosis, inflammation of the pancreas (pancreatitis), low blood sugar when used with insulin or medicines that stimulate insulin release, serious allergic reactions, reduced vitamin B12 levels, severe and disabling joint pain, and a blistering skin condition called bullous pemphigoid. Pancreatitis can be fatal. Allergic reactions can include anaphylaxis, swelling and peeling skin. Contact a doctor promptly if you develop severe joint pain, skin blisters or open sores, or suspect any serious reaction. Suspected pancreatitis, serious allergic reactions and suspected bullous pemphigoid require stopping treatment and prompt medical assessment. Blood tests are recommended to monitor for vitamin B12 deficiency. The label also warns about heart failure, which has been observed with two other medicines in the same drug class as linagliptin. People with risk factors for heart failure need assessment and monitoring.

When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take this medicine if you have severe kidney impairment (eGFR below 30 mL/min/1.73 m²), acute or chronic metabolic acidosis, including diabetic ketoacidosis, or an allergy to linagliptin, metformin or any other ingredient. Use should be avoided in people with liver impairment. Conditions requiring caution include the risk of lactic acidosis, pancreatitis, serious allergic reactions, severe joint pain and bullous pemphigoid, a blistering skin condition. Pancreatitis, including fatal cases, has been reported. If pancreatitis or bullous pemphigoid is suspected, or a serious allergic reaction occurs, the medicine should be stopped and medical care sought. Report new skin blisters or sores and severe joint pain promptly. Taking this medicine with insulin or medicines that stimulate insulin release can increase the risk of low blood sugar. Your prescriber may need to adjust those medicines. Metformin may lower vitamin B12 levels; blood counts should be checked annually and vitamin B12 levels every 2 to 3 years. People with risk factors for heart failure need careful assessment and monitoring. Heart failure has been observed with two other medicines in the same class as linagliptin. Older adults need more frequent kidney function checks. Premenopausal women should be advised of the potential for unintended pregnancy.
Interactions
Topiramate, zonisamide, acetazolamide and dichlorphenamide may increase the risk of lactic acidosis (a buildup of lactic acid in the blood). More frequent monitoring may be needed. Ranolazine, vandetanib, dolutegravir and cimetidine can slow the removal of metformin from the body and may also increase this risk; your doctor should assess the benefits and risks of using them together. Avoid excessive alcohol intake, as alcohol can increase metformin’s effects on how the body processes lactic acid. Insulin and medicines that increase insulin release, such as sulfonylureas, increase the risk of low blood sugar when used together with linagliptin and metformin. Your prescriber may need to adjust these medicines. Some medicines can raise blood sugar and make it harder to control. These include thiazides and other diuretics, corticosteroids, phenothiazines, thyroid medicines, estrogens, oral contraceptives, phenytoin, nicotinic acid, sympathomimetics, calcium channel blockers and isoniazid. Close blood sugar monitoring may be needed when these medicines are started or stopped, including monitoring for low blood sugar after stopping them. Rifampin and other strong inducers of P-glycoprotein or CYP3A4 may reduce linagliptin’s effectiveness. The label strongly recommends alternative treatments if these medicines are needed.
