FDA boxed warningEncephalopathy including wernicke's
Serious and fatal encephalopathy, including Wernicke's, has occurred in patients treated with INREBIC. Wernicke's encephalopathy is a neurologic emergency. Assess thiamine levels in all patients prior to starting INREBIC. Do not start INREBIC in patients with thiamine deficiency; replete thiamine prior to treatment initiation. While on treatment all patients should receive prophylaxis with daily oral thiamine and should have thiamine levels assessed as clinically indicated. If encephalopathy is suspected, immediately discontinue INREBIC and initiate parenteral thiamine. Monitor until symptoms resolve or improve and thiamine levels normalize.
About Inrebic
Overview
Inrebic is a prescription medicine containing fedratinib, a kinase inhibitor supplied as capsules. It treats adults with intermediate-2 or high-risk myelofibrosis, either primary myelofibrosis or myelofibrosis that develops after polycythemia vera or essential thrombocythemia. It works by blocking certain enzymes, particularly JAK2, involved in cell-growth signals that can be abnormally active in myelofibrosis.
What it treats
- Intermediate-2 or high-risk primary myelofibrosis in adults. - Intermediate-2 or high-risk secondary myelofibrosis in adults following polycythemia vera or essential thrombocythemia.
Safety information
Side effects
The most common side effects of Inrebic are diarrhea, nausea, anemia (low red blood cell count), and vomiting. Contact your doctor promptly about severe diarrhea, nausea or vomiting, or concerns about serious side effects. These include encephalopathy (a brain disorder), including Wernicke’s encephalopathy; anemia; low platelet counts; liver toxicity; raised levels of the enzymes amylase and lipase; uveitis (inflammation inside the eye); major cardiovascular events; blood clots; and new cancers. Monitoring for new cancers is particularly important for current or past smokers. Symptoms may also worsen after treatment is interrupted or stopped.

Interactions
Tell your doctor about medicines you take that fall into these groups: • Strong CYP3A4 inhibitors: your prescriber will need to reduce your Inrebic dose. • Strong or moderate CYP3A4 inducers: use with Inrebic should be avoided. • CYP3A4, CYP2C19 or CYP2D6 substrates, or OCT2 and MATE1/2-K substrates: your prescriber may need to adjust the doses of these other medicines. Your doctor can check whether your medicines belong to these groups.

Storage
