Myelosuppression can be severe and lead to fatal infections. Monitor blood counts prior to and at intervals after each treatment cycle.

About Ifosfamide
Overview
Ifosfamide is an alkylating chemotherapy medicine given by intravenous infusion. It is used in adults with germ cell testicular cancer, together with certain other cancer medicines, as third-line chemotherapy. After activation in the liver, it mainly works by forming links within and between DNA strands, which leads to cell death.
What it treats
Germ cell testicular cancer in adults, as third-line chemotherapy in combination with certain other approved anticancer medicines.
Safety information
Side effects
Common side effects include hair loss, nausea and vomiting, blood in the urine, low white blood cell counts, anemia, effects on the brain and nervous system, and infections. Contact a doctor promptly for blood in the urine or signs of infection, or if you develop symptoms affecting the brain or nervous system. Serious risks include reduced blood cell production, encephalopathy (brain dysfunction), kidney and urinary tract damage, and severe allergic reactions, including anaphylaxis. Other serious effects include abnormal heart rhythms, heart muscle damage, and lung inflammation or scarring. Heart and lung toxicity can be fatal. Blockage of small veins in the liver and secondary cancers can also occur. Blood counts, kidney function and urine tests need monitoring during treatment. Ifosfamide can harm an unborn baby and impair fertility in both males and females. Effective contraception is needed; discuss pregnancy risks and fertility concerns with your doctor.

Before taking this medicine
Ifosfamide must not be used if you have a known allergy to ifosfamide or a blockage that prevents urine from flowing out. Treatment requires monitoring because ifosfamide can suppress blood cell production and cause brain, kidney, urinary tract, heart and lung toxicity. Blood counts, blood chemistry and urine chemistry should be monitored. Heart problems, including abnormal rhythms and heart muscle damage, can be fatal. The risk is higher in people with existing heart disease or kidney impairment, and with other heart-toxic medicines or radiation treatment. Kidney impairment requires close monitoring for adverse reactions. Other serious risks include lung inflammation and scarring, secondary cancers, blockage of small veins in the liver, and severe allergic reactions. Ifosfamide can harm an unborn baby. Effective contraception is advised, and breastfeeding should be avoided. It can also impair fertility in both men and women.
Interactions
Tell your doctor about all medicines and supplements you take. Medicines that increase CYP3A4 enzyme activity (CYP3A4 inducers) may increase ifosfamide toxicity, so monitoring is needed. Medicines that block CYP3A4 activity (CYP3A4 inhibitors) may reduce ifosfamide’s effectiveness.

Storage
