About Ibrance
Overview
Ibrance contains palbociclib, a kinase inhibitor used with other medicines to treat adults with certain types of locally advanced or metastatic breast cancer. It is used for hormone receptor (HR)-positive, HER2-negative disease, with the combination chosen according to prior endocrine treatment and, in some cases, a PIK3CA mutation confirmed by an FDA-authorized test. It is also used with trastuzumab, with or without pertuzumab, and endocrine therapy as maintenance treatment for HR-positive, HER2-positive disease after induction treatment. It works by blocking CDK4 and CDK6, proteins involved in helping cells grow and divide.
What it treats
• HR-positive, HER2-negative advanced or metastatic breast cancer in adults, combined with an aromatase inhibitor as initial endocrine-based therapy, or with fulvestrant after disease progression following endocrine therapy. • Endocrine-resistant, PIK3CA-mutated, HR-positive, HER2-negative locally advanced or metastatic breast cancer in adults, combined with inavolisib and fulvestrant, following recurrence during or after adjuvant endocrine therapy. The PIK3CA mutation must be detected by an FDA-authorized test. • HR-positive, HER2-positive locally advanced or metastatic breast cancer in adults, as maintenance treatment following induction treatment, combined with trastuzumab, with or without pertuzumab, and endocrine therapy.
Safety information
Side effects
Common side effects depend on the medicines taken with Ibrance. They include low white blood cell counts, low hemoglobin and platelet counts, infections, tiredness, weakness, nausea, mouth inflammation or sores, diarrhea, hair loss, rash, itching, reduced appetite, headache and muscle spasms. Blood tests may also show increased creatinine, liver enzymes or fasting blood sugar, and low calcium, potassium, sodium or magnesium. Serious risks include neutropenia (a low count of neutrophils, a type of white blood cell) and lung inflammation called interstitial lung disease or pneumonitis, which can be severe or fatal. Contact your doctor promptly if you develop lung or breathing symptoms. Blood counts need to be checked before and during treatment. If lung inflammation is suspected, treatment must be interrupted; severe cases require permanent discontinuation. Ibrance can harm an unborn baby. Use effective contraception and contact your doctor if you become pregnant during treatment.

Before taking this medicine
The supplied FDA label lists no contraindications. However, Ibrance requires caution because it can cause neutropenia (a low count of certain white blood cells), severe or fatal lung inflammation, and harm to an unborn baby. Complete blood counts must be checked before treatment, at the start of each cycle, on day 15 of the first two cycles, and as clinically needed. Patients must be monitored for symptoms of lung inflammation. Treatment must be interrupted immediately if this is suspected and permanently stopped if it is severe. Patients should be advised of the risk to an unborn baby and to use effective contraception. Do not breastfeed during treatment.
Interactions
Ibrance interacts with medicines that strongly block or increase the activity of an enzyme called CYP3A. These combinations should be avoided. If a strong CYP3A blocker cannot be avoided, your prescriber will need to adjust your Ibrance treatment. Ibrance can also affect certain medicines processed by CYP3A4, particularly those for which small changes in exposure can cause harm. Your doctor may need to adjust those medicines. Ask your doctor to review your other medicines for these interactions.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
