FDA boxed warning

WARNING

RENAL IMPAIRMENT IS THE MAJOR TOXICITY OF FOSCAVIR. FREQUENT MONITORING OF SERUM CREATININE, WITH DOSE ADJUSTMENT FOR CHANGES IN RENAL FUNCTION, AND ADEQUATE HYDRATION WITH ADMINISTRATION OF FOSCAVIR IS IMPERATIVE. (See ADMINISTRATION section; Hydration. )

SEIZURES, RELATED TO ALTERATIONS IN PLASMA MINERALS AND ELECTROLYTES, HAVE BEEN ASSOCIATED WITH FOSCAVIR TREATMENT. THEREFORE, PATIENTS MUST BE CAREFULLY MONITORED FOR SUCH CHANGES AND THEIR POTENTIAL SEQUELAE. MINERAL AND ELECTROLYTE SUPPLEMENTATION MAY BE REQUIRED.

About Foscavir

Overview

Foscavir contains foscarnet sodium, an antiviral medicine. It comes in a vial as a sterile, preservative-free solution for administration into a vein only. It works by blocking a viral enzyme needed to copy viral DNA.

Safety information

Side effects

Frequently reported side effects in clinical trials included fever, nausea, anemia (low red blood cell count), diarrhea, vomiting and headache. These events were reported regardless of whether Foscavir caused them. Serious side effects that warrant contacting a doctor include impaired kidney function, changes in blood mineral levels, seizures, and bone marrow suppression (reduced blood cell production). Kidney impairment is Foscavir’s major toxicity. Blood mineral changes include low calcium, magnesium or potassium, and low or high phosphate. Seek urgent medical help if a seizure occurs. Kidney impairment, electrolyte changes and seizures may have contributed to deaths in clinical trials.

When to call your doctor

- Seek medical attention immediately if you have a seizure (fit).

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Foscavir must not be used in people with a clinically significant allergy to foscarnet sodium.

Interactions

Foscavir can interact with medicines that affect kidney function, blood calcium levels or heart rhythm. Tell your doctor if you take: • Medicines that can harm the kidneys, including aminoglycoside antibiotics, amphotericin B, cyclosporine, acyclovir, methotrexate, tacrolimus or intravenous pentamidine. These combinations should be avoided unless the potential benefits outweigh the risks. • Intravenous pentamidine or other medicines that affect blood calcium. Foscavir can lower blood calcium, and extreme caution is needed with these combinations. Kidney problems and symptomatic low calcium, including a fatal case of severe low calcium, have been reported with intravenous pentamidine. • Diuretics (“water tablets”). Loop diuretics may slow the removal of Foscavir from the body and increase the risk of toxicity. The label recommends thiazide diuretics over loop diuretics when a diuretic is needed. • Ritonavir, with or without saquinavir. Kidney problems have been reported during combined use with Foscavir. • Medicines that prolong the QT interval, a change in the heart’s electrical activity that can lead to a dangerous abnormal rhythm. These combinations should be avoided. Examples include quinidine, procainamide, dofetilide, amiodarone, sotalol, phenothiazines, tricyclic antidepressants, and certain macrolide and fluoroquinolone antibiotics.
