FDA boxed warningPost-treatment acute exacerbation of hepatitis b and risk of drug resistance with use of descovy for hiv-1 pre-exposure prophylaxis (prep) in undiagnosed early hiv-1 infection

Severe acute exacerbations of hepatitis B (HBV) have been reported in individuals with HBV who have discontinued products containing emtricitabine (FTC) and/or tenofovir disoproxil fumarate (TDF) and may occur with discontinuation of DESCOVY.

Hepatic function should be monitored closely with both clinical and laboratory follow-up for at least several months in individuals with HBV who discontinue DESCOVY. If appropriate, anti-hepatitis B therapy may be warranted.

DESCOVY used for HIV-1 PrEP must only be prescribed to individuals confirmed to be HIV-negative immediately prior to initiating and at least every 3 months during use. Drug-resistant HIV-1 variants have been identified with use of FTC/TDF for HIV-1 PrEP following undetected acute HIV-1 infection. Do not initiate DESCOVY for HIV-1 PrEP if signs or symptoms of acute HIV-1 infection are present unless negative infection status is confirmed.

About Descovy

Overview

Descovy is a tablet containing two antiretroviral medicines, emtricitabine and tenofovir alafenamide. It is used with other antiretroviral medicines to treat HIV-1 infection. It is also used for pre-exposure prophylaxis (PrEP) in at-risk adults and adolescents weighing at least 35 kg who test HIV-negative immediately before starting, to reduce the risk of acquiring HIV-1 through sex, excluding receptive vaginal sex, where effectiveness has not been evaluated. Both ingredients inhibit reverse transcriptase, an enzyme HIV uses to multiply.

What it treats

- HIV-1 infection, used with other antiretroviral medicines in adults and adolescents weighing at least 35 kg, and in children weighing 14 kg to less than 35 kg. For children in this weight range, companion medicines may include darunavir with cobicistat, but not other protease inhibitors that require a CYP3A inhibitor. - HIV-1 pre-exposure prophylaxis (PrEP) to reduce the risk of sexually acquired HIV-1 in at-risk adults and adolescents weighing at least 35 kg who test HIV-1 negative immediately before starting. This excludes people at risk through receptive vaginal sex, as effectiveness in this population has not been evaluated.

Safety information

Side effects

The most common side effect reported in participants with HIV-1 was nausea. In adults without HIV-1 taking Descovy for pre-exposure prophylaxis (PrEP), the most common side effect was diarrhea. Serious side effects that warrant contacting a doctor include severe flare-ups of hepatitis B, new or worsening kidney problems, lactic acidosis (a buildup of lactic acid in the blood), and severe liver enlargement with fat buildup. Immune reconstitution syndrome, an immune system reaction during HIV-1 treatment, may also require further evaluation and treatment.

Before taking this medicine

Descovy must not be used for HIV-1 pre-exposure prophylaxis (PrEP) if HIV-1 status is unknown or positive. Kidney function needs to be checked before starting and during use because new or worsening kidney problems may occur. People with chronic kidney disease also need blood phosphorus checks. Descovy should be stopped if symptoms or test results suggest lactic acidosis (a buildup of lactic acid in the blood) or serious liver toxicity, including severe liver enlargement with fat buildup. During HIV-1 treatment, immune reconstitution syndrome, an inflammatory response as the immune system recovers, may require further assessment and treatment. When used for PrEP, taking Descovy as prescribed and using safer sex practices, including condoms, are important to reduce the risk of sexually transmitted infections, including HIV-1. The risk of developing HIV-1 drug resistance also requires clinical management. Descovy is not recommended for HIV-1 treatment in children weighing less than 14 kg, or for PrEP in anyone weighing less than 35 kg.
