About Cytoxan
Overview
Cytoxan contains cyclophosphamide, an anticancer medicine available as tablets. It is used to treat certain lymphomas and leukemias, multiple myeloma, mycosis fungoides, neuroblastoma, retinoblastoma, and cancers of the ovary and breast. It is also used for biopsy-confirmed minimal change nephrotic syndrome in children when corticosteroid treatment has not worked adequately or cannot be tolerated. It is thought to work by forming links within tumour cell DNA.
What it treats
• Certain lymphomas: Hodgkin’s disease, lymphocytic lymphoma, mixed-cell lymphoma, histiocytic lymphoma and Burkitt’s lymphoma. • Multiple myeloma, leukemias and mycosis fungoides. • Neuroblastoma and retinoblastoma. • Ovarian adenocarcinoma and breast cancer. • Biopsy-confirmed minimal change nephrotic syndrome in children who have not responded adequately to corticosteroid treatment or cannot tolerate it. Safety and effectiveness for nephrotic syndrome in adults or other kidney diseases have not been established.
Safety information
Side effects
The most frequently reported side effects include low levels of neutrophils (a type of white blood cell), low neutrophils with fever, fever, hair loss, nausea, vomiting and diarrhea. Contact a doctor promptly for serious side effects, including allergic reactions; reduced blood cell production or bone marrow failure; severe weakening of the immune system and infections, which can be fatal; bladder or urinary tract inflammation, bleeding or kidney damage; heart inflammation, fluid around the heart, abnormal heart rhythms or heart failure; and lung inflammation or scarring that can lead to respiratory failure. Fever or blood in the urine warrants prompt medical contact. Other serious risks include new cancers, blockage of small veins in the liver that can be fatal, infertility, impaired wound healing and low blood sodium. Cyclophosphamide can harm an unborn baby; effective contraception is needed, and you should contact your doctor if pregnancy occurs.

Before taking this medicine
Do not take Cytoxan (cyclophosphamide) if you have had a severe allergic reaction to cyclophosphamide, its breakdown products or any other ingredient in the tablets, or if you have a blockage that prevents urine from flowing out normally. Severe allergic reactions, including fatal reactions, have been reported. Cross-allergy with other alkylating medicines can occur. Cyclophosphamide can suppress bone marrow and weaken the immune system, leading to serious and sometimes fatal infections. Close blood count monitoring is required. It can also cause urinary tract and kidney damage, including bladder inflammation with bleeding. Any urinary tract blockage must be ruled out or corrected before treatment. Other serious risks include heart damage, lung damage, new cancers and blockage of small veins in the liver. Some of these complications can be fatal. Monitoring is needed, particularly in people with existing heart disease or risk factors for heart damage. People with moderate or severe kidney impairment need monitoring for toxicity. Cyclophosphamide can harm an unborn baby and may impair fertility. Pregnancy status should be checked before treatment, and effective contraception should be used. Do not breastfeed during treatment.
Interactions
Cyclophosphamide can interact with other medicines and radiation treatment. Discuss these combinations with your doctor: • ACE inhibitors, natalizumab, thiazide diuretics and zidovudine may increase effects on blood cells or suppression of the immune system. Increased blood-cell toxicity has also been reported when cyclophosphamide is given after paclitaxel. • Protease inhibitors may increase levels of cyclophosphamide’s toxic breakdown products. Higher rates of infections, low neutrophil counts and inflammation of the mouth or digestive tract have been reported with certain combinations. • Anthracyclines, cytarabine, pentostatin, trastuzumab and radiation to the heart area may increase heart toxicity. • Amiodarone and medicines that stimulate white blood cell production, such as G-CSF or GM-CSF, may increase lung toxicity. • Amphotericin B may increase kidney toxicity. Acute water intoxication has been reported with indomethacin. • Azathioprine may increase liver toxicity. Busulfan has been associated with increased blockage of small liver veins and inflammation of the mouth or digestive tract. • Previous or current radiation treatment may increase the risk of bladder inflammation with bleeding. • Etanercept added to treatment including cyclophosphamide was associated with more solid cancers other than skin cancer in patients with Wegener’s granulomatosis. • Metronidazole has been linked to a report of acute brain dysfunction during cyclophosphamide treatment, although it is unclear whether the combination caused it. • Tamoxifen taken with chemotherapy may increase the risk of blood clots. • Warfarin’s effect may become stronger or weaker. • Cyclosporine levels may fall, potentially increasing the risk of graft-versus-host disease. Cyclophosphamide can prolong the breathing-stopping effects of certain muscle relaxants used during anesthesia, such as succinylcholine. Tell the anesthesiologist if you have received cyclophosphamide within the 10 days before general anesthesia.
