About Cotellic
Overview
Cotellic contains cobimetinib, a kinase inhibitor available as tablets. It is used with vemurafenib to treat adults with melanoma that cannot be removed by surgery or has spread and has a BRAF V600E or V600K mutation. It is also used alone to treat adults with histiocytic neoplasms. It works by blocking proteins called MEK1 and MEK2, which are part of a signalling pathway that promotes cell growth.
What it treats
- Melanoma in adults that cannot be removed by surgery or has spread to other parts of the body and has a BRAF V600E or V600K mutation, used in combination with vemurafenib. - Histiocytic neoplasms in adults, used on its own.
Safety information
Side effects
Common side effects reported with Cotellic vary by the condition being treated. In melanoma, they include diarrhoea, sensitivity to sunlight, nausea, fever and vomiting. In histiocytic neoplasms, they include acne-like rash, diarrhoea, infections, tiredness, nausea, swelling, dry skin, a rash with flat and raised spots, itching, indigestion, vomiting, shortness of breath and urinary tract infections. Serious risks include new cancers, including skin cancers; major bleeding; weakened heart muscle; severe skin reactions; fluid buildup under the retina or blockage of a retinal vein; liver damage; muscle breakdown; and severe sensitivity to sunlight. Contact a doctor promptly for signs of bleeding, severe skin rash, vision changes or signs of a new skin cancer. Avoid sun exposure during treatment. Blood tests may show serious changes in liver enzymes, muscle enzymes, kidney function markers, blood cell counts and mineral levels. Monitoring includes heart function checks, eye examinations and blood tests. Cotellic can harm an unborn baby. Use effective contraception if you could become pregnant, and contact your doctor if you become pregnant during treatment.

Before taking this medicine
The FDA label lists no formal contraindications for Cotellic. However, serious risks require careful assessment and monitoring. Caution is needed because Cotellic can cause major bleeding, new cancers, severe skin reactions, liver injury, severe muscle breakdown, eye problems and severe sensitivity to sunlight. Monitoring includes checks for new cancers before and during treatment and for up to 6 months after the last dose, along with liver tests, muscle enzyme tests and regular eye examinations. Any changes in vision need an eye assessment. Avoid sun exposure. The risk of damage to the heart muscle is higher when Cotellic is used with vemurafenib than with vemurafenib alone. Its safety has not been established in people with reduced heart-pumping function. Heart function should be checked before treatment, after one month and every three months during treatment. Vemurafenib also has separate serious risks that must be reviewed. Cotellic can harm an unborn baby. People who could become pregnant should use effective contraception. Do not breastfeed while taking Cotellic.
Interactions
Cotellic should not be taken with medicines that strongly or moderately increase or block the activity of an enzyme called CYP3A. Ask your doctor to check whether any medicines you take have this effect.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
