About Casodex

Overview

Casodex tablets contain bicalutamide, an androgen receptor inhibitor. It is used together with a luteinizing hormone-releasing hormone (LHRH) analog to treat prostate cancer that has spread to other parts of the body (Stage D2). It works by blocking the action of androgen hormones, which can help prostate cancer grow.

What it treats

Metastatic prostate cancer (Stage D2), in combination with a luteinizing hormone-releasing hormone (LHRH) analog.

Side effects

Common side effects reported in more than 10% of patients taking Casodex with an LHRH agonist included hot flashes, pain (including back, pelvic and abdominal pain), weakness, constipation, infection, nausea, swelling of the arms or legs, shortness of breath, diarrhoea, blood in the urine, urinating at night and anaemia. Breast enlargement and breast pain have also been reported when Casodex is used alone. Serious risks include severe liver injury and liver failure, which can be fatal. Contact your doctor if you develop symptoms suggesting liver problems. Liver blood tests are needed before and during treatment. Bleeding has been reported when Casodex is taken with coumarin anticoagulants. Contact your doctor if bleeding occurs; close monitoring of blood clotting is needed with this combination. LHRH agonists used with Casodex can reduce the body’s ability to control blood sugar. Your doctor may monitor your blood glucose.

When to call your doctor

• Seek medical attention immediately for trouble breathing. • Seek medical attention immediately if you notice blood in your urine. • Seek medical attention immediately for unusual or heavy bleeding, especially if you also take a coumarin blood thinner.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Casodex if you have had an allergic reaction to bicalutamide or any tablet ingredient. Reported allergic reactions include swelling beneath the skin and hives. Casodex must not be used by women, including during pregnancy, because it can harm an unborn baby. Use caution if you have liver problems. Severe liver injury and fatal liver failure have been reported. Liver blood tests are needed before treatment, regularly during the first four months and periodically thereafter. Tell your prescriber if you take a coumarin anticoagulant, as bleeding can occur with this combination. Close monitoring of blood clotting tests (PT/INR) is needed, and your prescriber may need to adjust the anticoagulant treatment. When Casodex is used with an LHRH agonist, blood glucose monitoring may be needed because LHRH agonists can reduce glucose tolerance. Men with female partners who could become pregnant should use effective contraception.

Interactions

Casodex may increase blood levels of medicines broken down by the liver enzyme CYP3A4, such as midazolam. Tell your doctor if you take these medicines, as caution is needed when they are used together. If you take a coumarin anticoagulant (a blood thinner), your doctor should closely monitor your PT/INR blood tests, which measure how long your blood takes to clot. Your doctor may need to adjust your anticoagulant treatment. Clinical studies have not shown interactions between bicalutamide and goserelin or leuprolide.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
