FDA boxed warningDiarrhea and myelosuppression
Early and late forms of diarrhea can occur. Early diarrhea may be accompanied by cholinergic symptoms which may be prevented or ameliorated by atropine. Late diarrhea can be life threatening and should be treated promptly with loperamide. Monitor patients with diarrhea and give fluid and electrolytes as needed. Institute antibiotic therapy if patients develop ileus, fever, or severe neutropenia. Interrupt CAMPTOSAR and reduce subsequent doses if severe diarrhea occurs.
Severe myelosuppression may occur.
About Camptosar
Overview
Camptosar contains irinotecan hydrochloride, an anticancer medicine given by intravenous infusion. It is used with fluorouracil and leucovorin as an initial treatment for cancer of the colon or rectum that has spread to other parts of the body. It is also used when this cancer has returned or progressed after initial fluorouracil-based treatment. It works by blocking an enzyme involved in copying DNA, causing DNA damage that cells cannot efficiently repair.
What it treats
- Metastatic cancer of the colon or rectum, as first-line treatment in combination with 5-fluorouracil and leucovorin. - Metastatic cancer of the colon or rectum that has returned or progressed after initial fluorouracil-based treatment.
Safety information
Side effects
Common side effects include nausea, vomiting, stomach pain, diarrhea, constipation, loss of appetite, weakness, fever, hair loss, and reduced red and white blood cell counts. Weight loss is also common when Camptosar is used alone. When used with other medicines, common effects also include mouth or digestive tract inflammation, pain, infection, low platelet counts and abnormal bilirubin levels. Contact your doctor promptly for severe diarrhea or vomiting, fever, signs of infection, or new or worsening shortness of breath or cough. Diarrhea can occur during or shortly after an infusion, or start more than 24 hours later. Severe vomiting or diarrhea can cause dehydration and, in rare cases, kidney problems or acute kidney failure. Camptosar can severely reduce blood cell production, including white blood cells that help fight infection. Certain inherited variations in the UGT1A1 gene increase the risk of severely low neutrophil counts. Serious lung disease, including fatal cases, has also occurred. Severe allergic reactions, including anaphylaxis, require immediate medical attention.

Before taking this medicine
Do not use Camptosar if you have a known allergy to irinotecan or any of its ingredients. It should not be used in patients on dialysis. Use requires caution in people with kidney or liver impairment. Certain inherited UGT1A1 gene variants increase the risk of a severe fall in neutrophils, a type of white blood cell. People over 65 need close monitoring because they have a higher risk of diarrhea. Camptosar can cause severe diarrhea, reduced blood cell production, serious allergic reactions and potentially fatal lung problems. Severe vomiting or diarrhea can cause dehydration and may lead to kidney failure. Camptosar can harm an unborn baby. Women who could become pregnant should use effective contraception, and male patients with female partners who could become pregnant should use condoms. Do not breastfeed during treatment.
Interactions
Camptosar must not be used with medicines that strongly increase or block the activity of CYP3A4, an enzyme involved in drug breakdown. Ask your doctor to check your other medicines for these interactions before treatment.

Storage
Store in the original carton below 25°C, protected from light, unless the pack states refrigeration. Do not freeze. Keep all medicines out of the sight and reach of children.
