About Cabometyx

Overview

Cabometyx is a prescription tablet containing cabozantinib (S)-malate, used to treat advanced kidney cancer, including as an initial treatment with nivolumab, and liver cancer previously treated with sorafenib. It is also used in adults and children aged 12 years and older for locally advanced or metastatic differentiated thyroid cancer that has progressed after VEGFR-targeted treatment, when radioactive iodine has not worked or is unsuitable. In the same age group, it treats previously treated, well-differentiated neuroendocrine tumours arising in or outside the pancreas that cannot be removed surgically and are locally advanced or metastatic. Cabozantinib blocks certain proteins called tyrosine kinases, which are involved in cancer growth, spread and the formation of blood vessels supplying tumours.

What it treats

• Advanced renal cell carcinoma (kidney cancer), including first-line treatment in combination with nivolumab. • Hepatocellular carcinoma (liver cancer) previously treated with sorafenib. • Locally advanced or metastatic differentiated thyroid cancer in adults and children aged 12 years and older, after progression following VEGFR-targeted therapy, when radioactive iodine has not worked or is unsuitable. • Previously treated, well-differentiated pancreatic or extra-pancreatic neuroendocrine tumors in adults and children aged 12 years and older, when the tumors cannot be surgically removed and are locally advanced or metastatic.

Side effects

Common side effects of Cabometyx taken alone include diarrhea, tiredness, hand-foot skin reactions, reduced appetite, high blood pressure, nausea, vomiting, weight loss and constipation. When used with nivolumab, common side effects include diarrhea, tiredness, liver toxicity, hand-foot skin reactions, mouth inflammation, rash, high blood pressure, an underactive thyroid, muscle and bone pain, reduced appetite, nausea, changes in taste, abdominal pain, cough and upper respiratory tract infections. Contact a doctor promptly for serious side effects, including bleeding; a hole or abnormal connection in the digestive tract; blood clots, including heart attack; severely high blood pressure; heart failure; severe diarrhea; or severe hand-foot skin reactions. Other serious risks include liver damage, reduced adrenal gland function when used with nivolumab, protein in the urine, jawbone damage, poor wound healing, thyroid problems, low blood calcium and a serious brain condition called reversible posterior leukoencephalopathy syndrome (RPLS). Cabometyx can harm an unborn baby. Effective contraception is needed for people who could become pregnant.

When to call your doctor

• Seek medical attention immediately for bleeding. • Get urgent medical help for severe diarrhea. • Contact your doctor immediately if a surgical wound is not healing.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no formal contraindications. However, Cabometyx should not be given to anyone with a recent history of bleeding and should be avoided in people with severe liver impairment. Moderate liver impairment requires adjustment of treatment by the prescriber. Conditions requiring caution and monitoring include high blood pressure, heart failure, blood clots, holes or abnormal connections in the digestive tract, severe diarrhea, hand–foot skin reactions, protein in the urine, jawbone damage, poor wound healing, thyroid problems and low blood calcium. A serious neurological condition called reversible posterior leukoencephalopathy syndrome requires treatment to be stopped. When Cabometyx is used with nivolumab, additional risks include liver injury and reduced adrenal gland function. Liver enzyme monitoring is required before and during this combination treatment. Treatment must be withheld around elective surgery and invasive dental procedures, as directed by the prescriber, because of wound-healing and jawbone risks. Adolescents with open growth plates need monitoring for abnormalities. Cabometyx can harm an unborn baby. Women who could become pregnant should use effective contraception. Do not breastfeed during treatment.

Interactions

Strong CYP3A4 inhibitors and strong or moderate CYP3A4 inducers can interact with Cabometyx. Ask your doctor to check whether any medicines you take fall into these groups. If taking them together cannot be avoided, your prescriber may need to adjust your Cabometyx dose.
