About Braftovi
Overview
Braftovi is a prescription capsule containing encorafenib, a kinase inhibitor. It is used with binimetinib for melanoma that cannot be removed by surgery or has spread and has a BRAF V600E or V600K mutation, and for metastatic non-small cell lung cancer in adults with a BRAF V600E mutation. For adults with metastatic colorectal cancer with a BRAF V600E mutation, it is used with cetuximab and fluorouracil-based chemotherapy, or with cetuximab after prior treatment; these uses require mutation confirmation with an FDA-authorized test. It works by blocking BRAF kinase activity involved in signals that can drive cancer cell growth.
What it treats
• Melanoma that cannot be removed by surgery or has spread, with a BRAF V600E or V600K mutation, in combination with binimetinib. • Metastatic colorectal cancer in adults with a BRAF V600E mutation, in combination with cetuximab and fluorouracil-based chemotherapy, or with cetuximab after prior treatment. • Metastatic non-small cell lung cancer in adults with a BRAF V600E mutation, in combination with binimetinib. The BRAF mutation must be confirmed by an FDA-authorized test. Braftovi is not indicated for these cancers when they have wild-type BRAF.
Safety information
Side effects
Common side effects depend on the medicines used with Braftovi: • With binimetinib for melanoma: tiredness, nausea, vomiting, abdominal pain and joint pain. • With cetuximab and mFOLFOX6 for colorectal cancer: nerve problems affecting the hands or feet, nausea, tiredness, diarrhoea, reduced appetite, rash, vomiting, bleeding, abdominal pain, joint pain, fever and constipation. • With cetuximab and FOLFIRI for colorectal cancer: nausea, diarrhoea, tiredness, vomiting, hair loss, constipation, abdominal pain, reduced appetite and rash. • With cetuximab for colorectal cancer: tiredness, nausea, diarrhoea, acne-like skin inflammation, abdominal pain, reduced appetite, joint pain and rash. • With binimetinib for non-small cell lung cancer: tiredness, nausea, diarrhoea, muscle and bone pain, vomiting, abdominal pain, vision problems, constipation, shortness of breath, rash and cough. Serious risks include new cancers of the skin or other organs, increased growth of tumours without a BRAF mutation, weakened heart muscle, liver injury, major bleeding, inflammation inside the eye and changes in the heart’s electrical activity called QT prolongation. Contact a doctor promptly about bleeding, changes in vision or concerns about these serious effects. Heart, liver, electrolyte, skin and eye checks are needed to monitor for risks that may not cause obvious symptoms. Braftovi can harm an unborn baby. Contact your doctor if you become pregnant during treatment. People who could become pregnant should use effective nonhormonal contraception.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
The FDA label lists no contraindications for Braftovi. However, important precautions apply: • Braftovi can cause new cancers, including skin cancers. Skin checks are needed before, during and after treatment, along with monitoring for other cancers. BRAF inhibitors can also increase tumour cell growth in tumours without a BRAF mutation. • When used with binimetinib, Braftovi can cause heart muscle problems, liver injury and major bleeding. Heart function and liver tests are needed before and during treatment. The safety of this combination has not been established in people with a left ventricular ejection fraction below 50%. • Braftovi can cause inflammation inside the eye. Regular eye examinations are needed, and any vision changes should be assessed. • Braftovi can prolong the QT interval, a change in the heart’s electrical activity. Electrolytes should be checked before and during treatment, and electrolyte abnormalities and heart-related risk factors should be managed. • Braftovi can harm an unborn baby. People who could become pregnant should use effective nonhormonal contraception. Do not breastfeed during treatment. Braftovi may impair male fertility. • Braftovi is used in combination with binimetinib or cetuximab. If binimetinib is interrupted or stopped, the prescriber must also review Braftovi treatment.
Interactions
Braftovi can interact with other medicines. Ask your doctor to review your medicines for these interactions: • Strong or moderate CYP3A4 inhibitors should be avoided with Braftovi. If combined use is unavoidable, your doctor will need to adjust your Braftovi treatment. • Strong CYP3A4 inducers should be avoided. • Braftovi can reduce the effectiveness of certain medicines processed by CYP3A4, including hormonal contraceptives. These combinations should be avoided when reduced effectiveness could be a concern. • Medicines that rely on the transporters OATP1B1, OATP1B3 or BCRP may need to be reduced by your doctor when taken with Braftovi.

Dosing

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
