Postmarketing reports indicate that the effects of BOTOX and all botulinum toxin products may spread from the area of injection to produce symptoms consistent with botulinum toxin effects. These may include asthenia, generalized muscle weakness, diplopia, ptosis, dysphagia, dysphonia, dysarthria, urinary incontinence and breathing difficulties. These symptoms have been reported hours to weeks after injection. Swallowing and breathing difficulties can be life threatening and there have been reports of death. The risk of symptoms is probably greatest in children treated for spasticity but symptoms can also occur in adults treated for spasticity and other conditions, particularly in those patients who have an underlying condition that would predispose them to the

About Botox
Overview
Botox is a prescription medicine containing onabotulinumtoxinA, a botulinum toxin type A, supplied as a powder in a vial for preparation before injection. It is used to prevent headaches in adults with chronic migraine and to treat muscle spasticity in people aged 2 years and older. It also treats overactive bladder and urinary leakage caused by nerve-related bladder overactivity in adults when anticholinergic medicines have not worked well enough or cannot be tolerated. Botox works by blocking the release of acetylcholine, a chemical messenger involved in muscle activity, bladder contractions and sweating.
What it treats
- Overactive bladder in adults with urgency, frequent urination and urge-related leakage when anticholinergic medication has not worked adequately or cannot be tolerated. - Urinary leakage caused by bladder muscle overactivity linked to a neurological condition in adults when anticholinergic medication has not worked adequately or cannot be tolerated. - Bladder muscle overactivity linked to a neurological condition in children aged 5 years and older when anticholinergic medication has not worked adequately or cannot be tolerated. - Prevention of headaches in adults with chronic migraine: at least 15 headache days per month, with headaches lasting at least 4 hours a day. - Spasticity in patients aged 2 years and older. - Cervical dystonia in adults, to reduce abnormal head positioning and neck pain. - Severe underarm sweating in adults that is not adequately controlled by topical treatments. - Eyelid spasms associated with dystonia in patients aged 12 years and older. - Strabismus (misaligned eyes) in patients aged 12 years and older.
Safety information
Side effects
Common side effects vary by the condition being treated: • Overactive bladder: urinary tract infection, painful urination and difficulty emptying the bladder. • Bladder overactivity associated with a neurological condition: urinary tract infection and difficulty emptying the bladder in adults; urinary tract infection, white blood cells in the urine and bacteria in the urine in children. • Chronic migraine: neck pain and headache. • Spasticity: pain in the arms or legs in adults; upper respiratory tract infection in children. • Cervical dystonia: difficulty swallowing, upper respiratory infection, neck pain, headache, increased cough, flu-like symptoms, back pain and inflammation inside the nose. • Excessive underarm sweating: injection-site pain and bleeding, sweating outside the underarms, sore throat and flu-like symptoms. Botox’s toxin effects can spread beyond the injection site. Swallowing and breathing difficulties can be fatal. Seek immediate medical attention if breathing, speech or swallowing difficulties occur. Other serious risks include allergic reactions, difficulty emptying the bladder, and urinary tract infections. Contact a doctor if these occur. Treatment for eyelid spasms can reduce blinking and cause damage or ulcers on the eye’s surface. Treatment for crossed eyes can cause bleeding behind the eye and impaired blood flow to the retina. People with existing neuromuscular disorders may have more pronounced treatment effects. Those with impaired breathing function require caution because of respiratory risks. Bronchitis and upper respiratory tract infections can occur during treatment for spasticity. Bladder treatment may require monitoring for incomplete bladder emptying, particularly in people with multiple sclerosis or diabetes who do not routinely use a catheter.

When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not receive Botox if you are allergic to any botulinum toxin product or any ingredient in Botox, or if you have an infection at a planned injection site. For injections into the bladder muscle, Botox must not be used if you have a urinary tract infection. It must also not be used if you have urinary retention or more than 200 mL of urine remaining in the bladder after urination, unless you routinely perform clean intermittent self-catheterization. Extra caution is needed if you have a neuromuscular disorder or reduced breathing function. Toxin effects can spread, and swallowing or breathing difficulties can be fatal. Seek immediate medical attention if you develop difficulty breathing, speaking or swallowing. Bladder treatment can cause urinary tract infections or urinary retention. If you do not routinely use a catheter, the amount of urine remaining after urination should be monitored during treatment for overactive bladder or adult bladder overactivity associated with a neurological condition, particularly if you have multiple sclerosis or diabetes. Treatment for eyelid spasms can reduce blinking and cause corneal exposure or ulcers. Treatment for misaligned eyes can cause bleeding behind the eye or impair blood flow to the retina. Bronchitis and upper respiratory tract infections have been reported in people treated for spasticity. Based on animal data, Botox may harm an unborn baby. Serious adverse reactions may occur with unapproved uses. Botox potency units are not interchangeable with those of other botulinum toxin products.
Interactions
Tell your doctor if you take aminoglycoside antibiotics, muscle relaxants or other medicines that affect communication between nerves and muscles, such as curare-like agents. These may increase the effects of Botox, so close monitoring is needed when they are used together.
