Safety information
Side effects
Common side effects include stomach pain, diarrhea, nausea, vomiting and changes in taste. Serious side effects include severe allergic reactions, liver injury, QT prolongation (a change in the heart’s electrical activity) and potentially dangerous heart rhythms, Clostridium difficile-associated diarrhea, and worsening of myasthenia gravis. Contact a doctor promptly if diarrhea occurs or myasthenia gravis worsens. Stop taking clarithromycin and seek immediate medical help if a severe allergic reaction or signs of hepatitis occur. Serious reactions can also occur when clarithromycin is taken with certain other medicines, including colchicine, some cholesterol-lowering medicines and some calcium channel blockers. In people with coronary artery disease, the label warns of a potential increased risk of death from any cause one year or more after treatment ends. Discuss this risk with the prescriber.

Interactions
Clarithromycin can slow the breakdown of many medicines, increasing their levels and side effects. Tell your doctor about your other medicines before starting treatment. Some combinations must not be used; others need closer monitoring or changes made by your prescriber. Do not take clarithromycin with lovastatin, simvastatin, lomitapide, ergotamine, dihydroergotamine, cisapride, pimozide or lurasidone. It must not be combined with colchicine if you have kidney or liver impairment. Colchicine also needs special caution in people with normal kidney and liver function. Important interactions to raise with your doctor include: • Heart medicines: Combining clarithromycin with disopyramide, quinidine, dofetilide, amiodarone, sotalol or procainamide is not recommended. Serious heart rhythm problems have been reported with quinidine and disopyramide. Clarithromycin can also increase digoxin levels, potentially causing serious toxicity. Heart rhythm or blood-level monitoring may be needed. • Warfarin and other oral blood thinners: Their effects may increase, so blood-clotting tests need careful monitoring. • Cholesterol medicines: Atorvastatin and pravastatin require caution, in addition to the prohibited combinations listed above. • Blood pressure medicines: Verapamil, amlodipine, diltiazem and nifedipine require caution. Low blood pressure has been reported with verapamil and nifedipine; slow or irregular heart rhythms have also been reported with verapamil. • Quetiapine: Levels may rise, increasing the risk of sleepiness, low blood pressure on standing, altered consciousness and other serious reactions. • Diabetes medicines: Insulin, nateglinide, pioglitazone, repaglinide and rosiglitazone require caution. • Carbamazepine, theophylline, cyclosporine and tacrolimus: Interactions may require monitoring, including blood-level checks where appropriate. • Itraconazole and certain antiviral medicines, including atazanavir, saquinavir, ritonavir, etravirine and maraviroc: These combinations require review because medicine levels may change. Clarithromycin tablets can reduce zidovudine levels when taken at the same time; ask your prescriber how to coordinate them. • Sildenafil, tadalafil and vardenafil: Clarithromycin increases exposure to these medicines, and taking them together is not recommended. • Tolterodine: Levels may rise substantially in people whose bodies have low activity of the enzyme CYP2D6.
