About Solu‑Cortef
Overview
Solu-Cortef is a prescription injectable corticosteroid containing hydrocortisone sodium succinate, supplied in a vial. When oral treatment is not feasible, it is used for conditions including adrenal hormone insufficiency, severe allergic conditions that have not responded to conventional treatment, and certain inflammatory disorders. It works by reducing inflammation.
What it treats
Solu‑Cortef is used when oral treatment is not feasible and injection is appropriate for conditions including: • Adrenal insufficiency, congenital adrenal hyperplasia, cancer-related high blood calcium and certain thyroid inflammation. • Severe allergic conditions that have not responded to usual treatment. • Severe skin disorders, including pemphigus and Stevens–Johnson syndrome. • Ulcerative colitis and regional enteritis during critical periods. • Certain blood disorders, including autoimmune hemolytic anemia. • Leukemias and lymphomas, for palliative management. • Brain swelling associated with brain tumors or brain surgery. • Certain inflammatory eye diseases and nephrotic syndrome. • Certain lung diseases, including symptomatic sarcoidosis. • Acute flare-ups of selected rheumatic conditions, including rheumatoid arthritis and gout, as short-term add-on treatment. • Systemic lupus erythematosus, dermatomyositis, polymyositis and temporal arteritis.
Safety information
Side effects
The label does not state how often side effects occur, so none can be identified as common. Reported effects include nausea, increased appetite, weight gain, fluid retention, headache, dizziness, trouble sleeping, mood changes, acne, increased sweating, bruising, thinning skin, slower wound healing and menstrual changes. Serious reported effects that warrant contacting a doctor include: - Severe allergic reactions, including anaphylaxis and swelling beneath the skin, which require immediate medical attention. - Heart rhythm disturbances, heart failure, cardiac arrest, blood clots and fluid in the lungs. - High blood pressure, potassium loss, raised blood sugar or increased need for diabetes medication. - Stomach or intestinal ulcers, bleeding or perforation, and inflammation of the pancreas. - Seizures, depression, other psychiatric disturbances or increased pressure within the skull. - Glaucoma, cataracts and other eye disorders. - Muscle weakness, osteoporosis, fractures, tendon rupture and bone damage. - Reduced resistance to infection and injection-site infections following non-sterile administration. - Suppression of normal adrenal gland function, particularly during illness, injury or surgery, and slowed growth in children. These effects have been reported with Solu-Cortef or other corticosteroids.
When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Solu‑Cortef must not be used in people with a systemic fungal infection or a known allergy to the medicine or any of its ingredients. It must not be given by injection into a muscle in people with idiopathic thrombocytopenic purpura. It must not be given intrathecally (into the fluid surrounding the spinal cord), as severe medical events have been reported with this route.
Interactions
Solu‑Cortef can interact with several medicines. Tell your doctor if you use: • Amphotericin B or medicines that lower potassium, such as diuretics: potassium may fall too low. Heart enlargement and congestive heart failure have been reported with amphotericin B and hydrocortisone. • Digitalis medicines: low potassium may increase the risk of an irregular heartbeat. • Warfarin: its blood-thinning effect may change, so frequent blood-clotting tests may be needed. • Diabetes medicines: corticosteroids can raise blood sugar, and your doctor may need to adjust your treatment. • Aspirin or other nonsteroidal anti-inflammatory drugs (NSAIDs): the risk of stomach and intestinal side effects increases. Aspirin requires particular caution in people with low blood prothrombin levels. • Cyclosporine: the effects of both medicines may increase. Seizures have been reported with combined use. • Anticholinesterase medicines for myasthenia gravis: combined use may cause severe muscle weakness. • Ketoconazole, macrolide antibiotics such as erythromycin, or estrogens, including oral contraceptives: these may increase corticosteroid levels or effects. • Barbiturates, phenytoin, carbamazepine, rifampin or cholestyramine: these may make the body clear corticosteroids faster. • Isoniazid: its blood levels may decrease. • Aminoglutethimide: this may reduce corticosteroid-related suppression of adrenal gland activity. Discuss planned vaccinations with your doctor. Prolonged corticosteroid treatment may reduce vaccine responses and allow some organisms in live vaccines to multiply more readily. Routine vaccinations should be deferred until treatment has ended, if possible. Also tell the clinician before a skin test, as corticosteroids may suppress the reaction.

Dosing

Storage
Store in the original carton below 25°C, protected from light, unless the pack states refrigeration. Do not freeze. Keep all medicines out of the sight and reach of children.
