About Solu‑Cortef
Overview
Solu-Cortef is an injectable glucocorticoid medicine containing hydrocortisone sodium succinate. When oral treatment is not feasible, it is used for conditions such as adrenal insufficiency, severe allergies that have not responded to standard treatment, and certain inflammatory conditions, including rheumatoid arthritis and ulcerative colitis. Its anti-inflammatory action helps reduce inflammation.
What it treats
When oral treatment is not feasible, Solu-Cortef injection is used for conditions including: - Adrenal insufficiency, congenital adrenal hyperplasia, cancer-related high blood calcium and nonsuppurative thyroiditis. - Severe allergic conditions that have not responded to conventional treatment, including asthma and drug hypersensitivity reactions. - Certain serious skin diseases, including pemphigus and Stevens-Johnson syndrome. - Ulcerative colitis and regional enteritis during critical periods. - Certain blood disorders, including autoimmune hemolytic anemia and selected platelet disorders. - Leukemias and lymphomas, for palliative management. - Brain swelling associated with brain tumors or surgery involving opening the skull. - Certain inflammatory eye conditions. - Certain forms of nephrotic syndrome, to increase urine output or reduce protein loss in urine. - Symptomatic sarcoidosis and certain other inflammatory lung diseases. - Acute flare-ups of certain rheumatic conditions, including rheumatoid arthritis and gouty arthritis, as short-term additional treatment. - Systemic lupus erythematosus, dermatomyositis, polymyositis and temporal arteritis.
Safety information
Side effects
Reported side effects of Solu-Cortef or other corticosteroids include nausea, increased appetite, weight gain, fluid retention, headache, dizziness, trouble sleeping, mood changes, increased sweating, acne, rash, bruising, thinning skin and slower wound healing. The label does not state how common these effects are. Serious reported side effects that warrant medical attention include: • Severe allergic reactions, including anaphylaxis and swelling beneath the skin. • Heart rhythm changes, heart failure, high blood pressure, blood clots and fluid in the lungs. • Stomach or intestinal ulcers, bleeding or perforation, and inflammation of the pancreas. • Reduced resistance to infection. • Increased blood sugar, increased need for diabetes medicines, reduced adrenal gland function and slowed growth in children. • Potassium loss, muscle weakness, osteoporosis, fractures and tendon rupture. • Seizures, depression, severe mental or mood changes, and increased pressure within the skull. • Glaucoma, cataracts and other eye problems that can affect vision. Seek urgent medical help for a severe allergic reaction. Contact a doctor promptly about other serious effects.
When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Solu‑Cortef must not be used in people with systemic fungal infections or a known allergy to the medicine or any of its ingredients. It must not be injected into a muscle in people with idiopathic thrombocytopenic purpura, a condition involving a low platelet count. It must not be given intrathecally (into the fluid surrounding the spinal cord). Severe medical events have been reported with this route of administration.
Interactions
Solu-Cortef can interact with other medicines. Tell your doctor if you use: • Diuretics or amphotericin B: the combination can cause low potassium. Heart enlargement and congestive heart failure have been reported with amphotericin B and hydrocortisone. • Digitalis heart medicines: low potassium can increase the risk of an irregular heartbeat. • Warfarin: its blood-thinning effect may change, so frequent blood-clotting tests may be needed. • Diabetes medicines: Solu-Cortef can raise blood sugar, and your doctor may need to adjust your diabetes treatment. • Aspirin or other nonsteroidal anti-inflammatory drugs (NSAIDs): the risk of stomach and intestinal side effects increases. Aspirin needs particular caution in people with low levels of the clotting protein prothrombin. • Cyclosporine: the effects of both medicines may increase. Seizures have been reported with this combination. • Anticholinesterase medicines for myasthenia gravis: using them together may cause severe muscle weakness. • Ketoconazole, macrolide antibiotics such as erythromycin, or estrogens, including oral contraceptives: these may increase corticosteroid levels or effects. • Barbiturates, phenytoin, carbamazepine, rifampin or cholestyramine: these may increase how quickly the body breaks down or clears corticosteroids. • Isoniazid: its blood levels may decrease. • Aminoglutethimide: it may reduce corticosteroid-related suppression of the adrenal glands. Discuss vaccinations with your doctor. Prolonged corticosteroid treatment may reduce vaccine responses, and live vaccines may carry an increased risk of infection. Routine vaccination should be postponed until treatment has ended where possible. Tell the clinician arranging any skin tests that you are receiving Solu-Cortef, as it may suppress test reactions.
Using Solu‑Cortef
Dosing

Storage
Store in the original carton below 25°C, protected from light, unless the pack states refrigeration. Do not freeze. Keep all medicines out of the sight and reach of children.
