About Kapidex , Dexilant

Overview

Dexlansoprazole is a proton pump inhibitor (PPI) supplied as delayed-release capsules. It is used in adults and children aged 12 years and older to heal erosive esophagitis (damage to the lining of the food pipe), maintain healing and relieve heartburn. It also treats symptoms of non-erosive gastroesophageal reflux disease (GERD), where reflux occurs without erosion of the food pipe lining. It works by blocking the final step of acid production in the stomach.

What it treats

For patients aged 12 years and older: - Healing erosive esophagitis of all grades. - Maintaining healing of erosive esophagitis and relieving heartburn. - Treating symptomatic non-erosive gastroesophageal reflux disease (GERD).

Safety information

Side effects

Common side effects in adults include diarrhea, stomach pain, nausea, upper respiratory tract infection, vomiting and gas. In patients aged 12 to 17 years, common side effects include headache, stomach pain, diarrhea, inflammation of the nose and throat, and throat pain. Contact a doctor if you develop a serious side effect. These include kidney inflammation, diarrhea associated with Clostridium difficile infection, severe skin reactions or other signs of an allergic reaction, and new or worsening lupus affecting the skin or other parts of the body. Stop taking dexlansoprazole and seek medical advice at the first signs of a severe skin reaction or allergic reaction. Other serious risks include fractures of the hip, wrist or spine, particularly with long-term use; vitamin B12 deficiency with long-term daily use; and low magnesium or other mineral disturbances with prolonged treatment. Polyps in the stomach lining become more likely with long-term use, especially beyond one year. Dexlansoprazole is not recommended for children younger than 2 years because of the risk of heart valve thickening.

When to call your doctor

• Seek immediate medical attention for a severe skin reaction or other signs of an allergic reaction. • Contact your doctor if you develop diarrhea; dexlansoprazole may increase the risk of diarrhea associated with a bowel infection called Clostridium difficile.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take dexlansoprazole if you are allergic to any ingredient in the capsules or if you take any medicine containing rilpivirine. Allergic reactions can be severe. Important cautions include: • Kidney inflammation (acute tubulointerstitial nephritis): treatment should be stopped and the patient evaluated if this occurs. • Severe skin reactions or signs of an allergic reaction: stop treatment at the first signs and seek medical evaluation. • New or worsening lupus: stop treatment and seek specialist evaluation. • An increased risk of Clostridium difficile-associated diarrhea. • A possible increased risk of osteoporosis-related hip, wrist or spine fractures with long-term use. • Vitamin B12 deficiency, low magnesium and other mineral disturbances with prolonged use. • Fundic gland polyps, with risk increasing during long-term use, especially beyond one year. Treatment should be for the shortest duration needed. Improvement in symptoms does not rule out stomach cancer in adults; further assessment may be needed. Dexlansoprazole can also interfere with tests for neuroendocrine tumors. Tell your prescriber if you take methotrexate. Dexlansoprazole may increase or prolong methotrexate levels and cause toxicity; temporary interruption may be considered during high-dose methotrexate treatment. During pregnancy, animal data suggest a potential risk to fetal bone growth and development. Dexlansoprazole is not recommended for children under two years because of the risk of heart valve thickening. Based on lansoprazole data, it is not effective for symptomatic GERD in infants aged one month to under one year.

Interactions

Do not take dexlansoprazole with medicines containing rilpivirine. Avoid taking it with nelfinavir, rifampin or St. John’s wort. These combinations can reduce the effectiveness of dexlansoprazole or the antiviral medicine. Tell your doctor if you take: • Other HIV medicines, including atazanavir, saquinavir or ritonavir-containing products. Dexlansoprazole can affect some HIV medicines, reducing their effectiveness or increasing toxicity. • Warfarin. Blood-clotting test results may change, increasing the risk of serious bleeding. Monitoring is needed. • Methotrexate. Dexlansoprazole may increase methotrexate levels and toxicity. Your doctor may consider temporarily stopping dexlansoprazole. • Digoxin or tacrolimus. Blood levels may increase and need monitoring. • Iron salts, erlotinib, dasatinib, nilotinib, mycophenolate mofetil, ketoconazole or itraconazole. Reduced stomach acidity can lower their absorption. Particular caution is needed with mycophenolate mofetil in transplant patients. • Voriconazole. It may increase dexlansoprazole levels. Tell the clinician arranging any tests that you take dexlansoprazole. It can cause misleading results in chromogranin A blood tests and secretin stimulation tests used to investigate certain tumors. Your doctor may arrange a temporary interruption before testing. False-positive urine screening results for THC have also been reported; a positive result may need confirmation using another method.
