Before taking this medicine

Do not take Inspra (eplerenone) if you have a blood potassium level above 5.5 mEq/L before starting treatment, a creatinine clearance of 30 mL/min or less, or are taking a strong CYP3A inhibitor such as ketoconazole, itraconazole, nefazodone, troleandomycin, clarithromycin, ritonavir or nelfinavir. If Inspra is being used to treat high blood pressure, you must also not take it if you have: • Type 2 diabetes with small amounts of albumin in your urine (microalbuminuria). • A blood creatinine level above 2.0 mg/dL in males or above 1.8 mg/dL in females. • A creatinine clearance below 50 mL/min. • Ongoing treatment with potassium supplements or potassium-sparing diuretics, such as amiloride, spironolactone or triamterene. Inspra can cause high blood potassium (hyperkalemia). Extra caution is needed if you have reduced kidney function, diabetes or protein in your urine, or if you take ACE inhibitors, angiotensin receptor blockers (ARBs), nonsteroidal anti-inflammatory drugs (NSAIDs) or moderate CYP3A inhibitors. Your prescriber should monitor your blood potassium and adjust treatment as needed.
