FDA boxed warningCardiomyopathy, secondary malignancies, and extravasation and tissue necrosis
Cardiomyopathy: IDAMYCIN PFS can cause myocardial damage, including acute left ventricular failure, during or after termination of therapy. The risk of cardiomyopathy is increased in patients who have received prior anthracyclines or who have pre-existing cardiac disease. Assess left ventricular cardiac function prior to initiation of IDAMYCIN PFS and during and after treatment.
Secondary Malignancies: Secondary acute myelogenous leukemia (AML) and myelodysplastic syndrome (MDS) occur at a higher incidence in patients treated with anthracyclines, including IDAMYCIN PFS.
Extravasation and Tissue Necrosis: Extravasation of IDAMYCIN PFS during administration can result in local tissue injury and necrosis. Immediately terminate the infusion of IDAMYCIN PFS and institute the recommended management procedures.
Cardiomyopathy: Myocardial damage leading to congestive heart failure can occur with IDAMYCIN PFS. Assess left ventricular cardiac function prior to initiation of IDAMYCIN PFS and during and after treatment.
Extravasation of IDAMYCIN PFS during administration can result in local tissue injury and necrosis. Immediately discontinue the IDAMYCIN PFS infusion if extravasation occurs.
About Idamycin PFS
Overview
Idamycin PFS contains idarubicin hydrochloride, an anthracycline chemotherapy medicine. It is used with other chemotherapy medicines to treat adults with acute myeloid leukemia (AML). It works by damaging DNA and interfering with processes cells need to divide.
What it treats
Acute myeloid leukemia (AML) in adults, as part of a combination chemotherapy regimen.
Safety information
Side effects
Common side effects include infections, nausea and vomiting, hair loss, abdominal pain and diarrhea, bleeding, inflammation of the mouth and other mucous membranes, skin problems, changes in mental state, and lung problems. Serious side effects that warrant contacting a doctor include heart muscle damage, new cancers, severe tissue damage if the medicine leaks outside a vein, and severe suppression of blood cell production. This suppression can lead to severe infection, septic shock, bleeding, or death. Other serious risks include tumor lysis syndrome and hypersensitivity reactions. Blood counts, blood chemistry, kidney function, and liver function need monitoring during treatment. Idamycin PFS can harm an unborn baby. Use effective contraception and contact your doctor if you become pregnant during treatment.

Before taking this medicine
The label lists no formal contraindications. However, Idamycin PFS should be avoided in patients with serum bilirubin above 5 mg/dL, a marker of liver impairment. Treatment requires close monitoring because it can severely reduce blood cell counts, leading to serious infection, septic shock, bleeding or death. Complete blood counts are needed before each treatment. Kidney and liver function should be checked before and during treatment, with particular caution in patients with kidney or liver impairment. Patients also need monitoring for tumor lysis syndrome and hypersensitivity reactions. Idamycin PFS can harm an unborn baby. Patients should use effective contraception and be advised of the risk to a fetus. Do not breastfeed during treatment. The medicine may impair fertility.

Dosing
