FDA boxed warningSpinal/epidural hematomas

Epidural or spinal hematomas may occur in patients who are anticoagulated with low molecular weight heparins (LMWH) or heparinoids and are receiving neuraxial anesthesia or undergoing spinal puncture. These hematomas may result in long-term or permanent paralysis. Consider these risks when scheduling patients for spinal procedures. Factors that can increase the risk of developing epidural or spinal hematomas in these patients include:

Use of indwelling epidural catheters

Concomitant use of other drugs that affect hemostasis, such as non-steroidal anti-inflammatory drugs (NSAIDs), platelet inhibitors, other anticoagulants.

A history of traumatic or repeated epidural or spinal punctures

A history of spinal deformity or spinal surgery

Optimal timing between the administration of FRAGMIN and neuraxial procedures is not known

Monitor patients frequently for signs and symptoms of neurological impairment. If neurological compromise is noted, urgent treatment is necessary.

Consider the benefits and risks before neuraxial intervention in patients anticoagulated or to be anticoagulated for thromboprophylaxis.

About Fragmin

Overview

Fragmin (dalteparin sodium) is a low molecular weight heparin, a medicine that helps prevent and treat certain blood clots. It is used to prevent clot-related complications in unstable angina and non-Q-wave heart attacks, and to prevent deep vein thrombosis during abdominal or hip replacement surgery or acute illness with severely restricted mobility. It is also used for extended treatment of symptomatic blood clots in veins to reduce recurrence in adults with cancer, and to treat these clots and reduce recurrence in children from birth who were born at a gestational age of at least 35 weeks. It works by helping a natural clotting inhibitor block clotting proteins, mainly Factor Xa, to reduce clot formation.

What it treats

• Prevention of complications caused by reduced blood flow to the heart in unstable angina or non-Q-wave heart attack. • Prevention of deep vein thrombosis (DVT) in patients undergoing abdominal or hip replacement surgery, or with severely restricted mobility during acute illness. • Extended treatment of symptomatic venous thromboembolism (VTE) to reduce recurrence in adults with cancer. • Treatment of symptomatic VTE to reduce recurrence in children from birth, provided gestational age is at least 35 weeks. The label states that Fragmin is not indicated for acute treatment of VTE.

Side effects

Common side effects include bleeding, a low platelet count, bruising or pain at the injection site, and temporary increases in liver enzymes measured by blood tests. Contact a doctor if you experience bleeding or are told your platelet count is low. Serious bleeding, including bleeding around the spinal cord (spinal or epidural hematoma), can occur. Any decrease in platelets requires close monitoring. Periodic blood counts are recommended during treatment. Multiple-dose formulations contain benzyl alcohol and must not be used in newborns or infants.

- Seek immediate medical attention for bleeding. - Seek immediate medical attention for new difficulty moving or loss of movement, especially after an epidural, spinal anaesthesia or spinal puncture.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not use Fragmin if you have active major bleeding, a history of heparin-induced thrombocytopenia (a low platelet count caused by heparin), with or without blood clots, or a known allergy to dalteparin sodium, heparin or pork products. If you are undergoing epidural or neuraxial anesthesia, Fragmin must not be used to treat unstable angina or non-Q-wave heart attack, or for prolonged prevention of blood clots in veins. Use caution if you have a condition that increases your risk of bleeding. Any low platelet count requires close monitoring, and periodic blood counts are recommended. Multiple-dose formulations contain benzyl alcohol and must not be used in newborns or infants.

Interactions

Using Fragmin with oral anticoagulants (blood thinners taken by mouth), medicines that inhibit platelets, or medicines that dissolve blood clots may increase the risk of bleeding. Tell your doctor if you take any of these medicines.
