Cigarette Smoking and Serious Cardiovascular Events

Cigarette smoking increases the risk of serious cardiovascular events from hormonal contraceptive use. This risk increases with age, particularly in women over 35 years of age, and with the number of cigarettes smoked. For this reason, CHCs, including GWYN LO, are contraindicated in women who are over 35 years of age and smoke.

GWYN LO is contraindicated in women over 35 years old who smoke. Cigarette smoking increases the risk of serious cardiovascular events from combination hormonal contraceptives (CHC) use.

GWYN LO is contraindicated for use in women with a BMI ≥ 30 kg/m 2. Women with a BMI ≥ 30 kg/m 2 who use transdermal combined hormonal contraceptives may have a higher risk of venous thromboembolic events compared with women with a lower BMI.

About Estalis

Overview

Estalis is a prescription hormone replacement therapy patch containing estradiol and norethindrone. It is used to relieve menopausal symptoms, such as hot flushes and night sweats, in women who still have a uterus. Estradiol replaces estrogen that declines during menopause, while norethindrone helps protect the lining of the uterus from excessive thickening caused by estrogen.

- Menopausal symptoms, such as hot flushes and night sweats, in women who still have a uterus. - Prevention of osteoporosis after menopause in women at increased fracture risk when other treatments are unsuitable.

Safety information

Side effects

Common side effects include skin redness or irritation where the patch is applied, breast tenderness, headache, nausea, bloating, abdominal discomfort, and irregular vaginal bleeding or spotting. Serious risks include blood clots, stroke, heart attack and breast cancer. Seek emergency medical help for sudden chest pain, shortness of breath, coughing up blood, painful swelling in one leg, sudden weakness on one side, difficulty speaking, or sudden loss of vision. Severe allergic reactions, including swelling of the face, lips or tongue or difficulty breathing, also require emergency help. Contact a doctor promptly for a new breast lump, yellowing of the skin or eyes, a new severe headache, or vaginal bleeding that is heavy, persistent or starts after a period without bleeding.

When to call your doctor

- Chest pain, sudden shortness of breath or coughing up blood. - New pain, swelling, warmth or redness in one leg. - Sudden weakness or numbness on one side, trouble speaking, loss of vision or a sudden severe headache. - Yellowing of the skin or eyes, especially with dark urine or severe abdominal pain. - Swelling of the face, lips, tongue or throat, or difficulty breathing or swallowing.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Estalis should not be used by anyone with: • Known, suspected or previous breast cancer, or another known or suspected oestrogen-dependent cancer. • Unexplained vaginal bleeding or untreated thickening of the womb lining (endometrial hyperplasia). • Current or previous blood clots in the legs or lungs, or a known inherited blood-clotting disorder. • Current or recent arterial clotting disease, such as angina, a heart attack or a stroke. • Active liver disease, or previous liver disease if liver function has not returned to normal. • Porphyria. • An allergy to estradiol, norethindrone or any patch ingredient. Estalis is not intended for use during pregnancy or breastfeeding. Careful medical assessment is needed with high blood pressure, diabetes, migraine or severe headaches, fibroids, endometriosis, gallstones, high triglycerides, lupus, epilepsy, asthma, or heart or kidney problems. Additional caution is needed with a strong family history of breast cancer or blood clots, obesity, or other factors that increase clotting risk. Tell the prescriber about planned major surgery or prolonged immobility, as treatment may need to be interrupted. New or persistent vaginal bleeding requires medical assessment. Stop using Estalis and seek prompt medical advice if jaundice, a substantial rise in blood pressure or a new migraine-type headache develops. Seek urgent medical help for symptoms of a blood clot, heart attack or stroke.

Interactions

Tell your doctor about all medicines and supplements you take before starting Estalis, including herbal products. Medicines such as rifampicin, carbamazepine, phenytoin and phenobarbital, and the herbal remedy St John’s wort, can reduce hormone levels. This may make Estalis less effective or cause changes in vaginal bleeding. Some antifungal medicines, such as ketoconazole and itraconazole, and antibiotics such as clarithromycin can increase hormone levels and the risk of side effects. Grapefruit and grapefruit juice may also increase hormone levels. Some medicines used to treat HIV can alter hormone levels, so your doctor should check the specific combination. Check with your doctor or pharmacist before starting or stopping any medicine or supplement.

Storage

Store below 25°C flat in the original sealed sachet, away from direct light and heat, until you are ready to apply it. Keep all medicines out of the sight and reach of children.
