The Women's Health Initiative (WHI) estrogen-alone substudy reported increased risks of stroke and deep vein thrombosis (DVT) in postmenopausal women (50 to 79 years of age) during 7.1 years of treatment with daily oral conjugated estrogens (CE) [0.625 mg]-alone, relative to placebo.
The WHI Memory Study (WHIMS) estrogen-alone ancillary study of the WHI reported an increased risk of developing probable dementia in postmenopausal women 65 years of age and older during 5.2 years of treatment with daily CE (0.625 mg)-alone, relative to placebo. It is unknown whether this finding applies to younger postmenopausal women.
Prescribe estrogens with or without progestogens at the lowest effective doses and for the shortest duration consistent with treatment goals and risks for the individual woman.

About Climara
Overview
Climara is a patch containing estradiol, an estrogen hormone. It treats moderate to severe menopausal hot flashes and symptoms of thinning and dryness of the vulva and vagina, as well as low estrogen caused by underactive ovaries, surgical removal of the ovaries or primary ovarian failure. It is also used to prevent osteoporosis after menopause in women at significant risk, although non-estrogen medicines should be considered first; for vaginal and vulvar symptoms alone, topical vaginal products should be considered first. The patch continuously releases estradiol through intact skin, where it enters the body and binds to estrogen receptors in responsive tissues.
What it treats
- Moderate to severe hot flashes due to menopause. - Moderate to severe symptoms of vulvar and vaginal atrophy due to menopause. When these are the only symptoms being treated, topical vaginal products should be considered first. - Low estrogen levels due to hypogonadism, surgical removal of the ovaries or primary ovarian failure. - Prevention of osteoporosis after menopause. When used solely for prevention, non-estrogen medicines should be considered first, and estrogen therapy should be considered only for women at significant risk of osteoporosis.
Safety information
Side effects
Common side effects include breast pain, upper respiratory tract infections, headache, abdominal pain, other pain and swelling caused by fluid retention. Serious risks include heart and blood vessel disorders, cancers and gallbladder disease. Contact a doctor promptly if you develop any of these conditions. Stop using Climara and contact a doctor immediately if you experience loss of vision or develop severely elevated blood calcium, severely elevated triglycerides (blood fats), or cholestatic jaundice (yellowing of the skin or eyes caused by impaired bile flow).

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not use Climara if you have unexplained abnormal genital bleeding; breast cancer or a history of breast cancer; an estrogen-dependent tumour; blood clots in the deep veins or lungs, now or in the past; or a current or previous blood clot in an artery, such as a stroke or heart attack. It must not be used if you have liver disease or impaired liver function, protein C, protein S or antithrombin deficiency, or another known disorder that increases blood clotting. Do not use it if you have a known allergy to Climara, including a previous severe allergic reaction or angioedema (swelling beneath the skin). Estrogens increase the risk of gallbladder disease. Stop using Climara and seek medical advice if you develop severely elevated blood calcium, loss of vision, severely elevated triglycerides (blood fats), or jaundice caused by impaired bile flow. If you take thyroid hormone replacement, your thyroid function should be monitored.
Interactions
St. John’s wort, phenobarbital, carbamazepine and rifampin may lower estrogen levels, potentially reducing Climara’s effect or changing uterine bleeding patterns. Erythromycin, clarithromycin, ketoconazole, itraconazole, ritonavir and grapefruit juice may raise estrogen levels and increase the risk of side effects. Tell your doctor if you take any of these medicines or supplements, or consume grapefruit juice.
Using Climara
Dosing
Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.
Storage
Store below 25°C flat in the original sealed sachet, away from direct light and heat, until you are ready to apply it. Keep all medicines out of the sight and reach of children.
