About Cervidil
Overview
Cervidil is a vaginal insert containing dinoprostone, a prostaglandin analog. It is used to start or continue cervical ripening in pregnant women at or near term when there is a medical or obstetrical reason to induce labor. It works locally on the cervix, helping it soften, thin and open in preparation for labor.
What it treats
To start or continue preparing the cervix for labor in pregnant women at or near term when labor induction is needed for medical or obstetric reasons.
Safety information
Side effects
Common side effects include overly frequent uterine contractions, with or without signs of distress in the baby, and distress in the baby without overly frequent contractions. Serious effects that require immediate medical attention include overly frequent, prolonged or excessively strong uterine contractions; disseminated intravascular coagulation (a serious blood-clotting disorder); and amniotic fluid embolism syndrome. Signs of amniotic fluid embolism syndrome can include low blood pressure, low blood oxygen, breathing failure, blood-clotting problems, coma or seizures. Medical staff should monitor uterine activity and remove the vaginal insert if excessive contractions occur.
When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Cervidil must not be used if you: • Have a known allergy to prostaglandins. • Have known or suspected fetal distress when delivery is not imminent. • Have unexplained vaginal bleeding during the current pregnancy. • Have a known or suspected significant mismatch between the baby’s size and your pelvis. • Have a condition that makes inducing labour or using medicines that stimulate uterine contractions unsuitable. • Have had a previous caesarean section or other uterine surgery expected to affect the integrity of the uterus, such as surgery to remove fibroids. • Have a condition in which prolonged uterine contractions could endanger the baby. • Are receiving intravenous medicines that stimulate uterine contractions. • Have had six or more previous pregnancies carried to term. A history of glaucoma requires caution; the clinician should consider cervical-ripening methods that do not use prostaglandins. Uterine activity must be monitored. The vaginal insert must be removed if contractions become too frequent, excessively strong or prolonged. Clinicians should also assess for abnormal blood clotting and clot breakdown immediately after birth and monitor for signs of amniotic fluid embolism syndrome, including low blood pressure, low blood oxygen, respiratory failure, abnormal clotting, coma or seizures. Supportive care is required if this syndrome occurs.
Interactions
Cervidil may increase the effects of oxytocic medicines. It must not be used at the same time as intravenous oxytocic medicines. The vaginal insert must be removed at least 30 minutes before an oxytocic medicine is given. Tell your doctor if you are receiving any of these medicines.
