Cardiovascular Thrombotic Events
CELEBREX is contraindicated in the setting of coronary artery bypass graft (CABG) surgery.
Gastrointestinal Bleeding, Ulceration, and Perforation
NSAIDs cause an increased risk of serious gastrointestinal (GI) adverse events including bleeding, ulceration, and perforation of the stomach or intestines, which can be fatal. These events can occur at any time during use and without warning symptoms. Elderly patients and patients with a prior history of peptic ulcer disease and/or GI bleeding are at greater risk for serious GI events.
About Celebrex
Overview
Celebrex contains celecoxib, a nonsteroidal anti-inflammatory drug (NSAID) supplied as capsules. It is used for osteoarthritis, rheumatoid arthritis, ankylosing spondylitis and juvenile rheumatoid arthritis in patients aged 2 years and older. It is also used for acute pain and primary dysmenorrhea (menstrual pain). It is believed to work mainly by blocking an enzyme called COX-2, reducing the production of substances involved in pain and inflammation.

Safety information
Side effects
Common side effects reported in arthritis trials include stomach pain, diarrhea, indigestion, gas, swelling of the hands or feet, dizziness, sore throat, runny or stuffy nose, sinusitis, upper respiratory tract infection and rash. Accidental injury was also commonly reported. Serious side effects include cardiovascular problems caused by blood clots; bleeding, ulcers or holes in the stomach or intestines; liver injury; high blood pressure; heart failure and fluid retention; kidney damage; high blood potassium; and blood problems, including anemia. Contact a doctor promptly if you develop signs or symptoms of these problems. Stop taking Celebrex and contact a doctor at the first sign of a skin rash or other signs of an allergic reaction. Serious skin reactions and DRESS, a potentially serious reaction that can affect multiple parts of the body, may occur. Seek emergency medical help if a severe allergic reaction occurs.

Before taking this medicine
Do not take Celebrex if you are allergic to celecoxib, any of its ingredients or sulfonamides, or if you have had asthma, hives or another allergic reaction after taking aspirin or another nonsteroidal anti-inflammatory drug (NSAID). It must not be used in connection with coronary artery bypass graft (CABG) surgery. Tell your prescriber if you have high blood pressure, heart failure, kidney or liver problems, dehydration, low blood volume, asthma or signs of anemia. Blood pressure, kidney function or blood counts may need monitoring. Celebrex should be avoided in severe heart failure or advanced kidney disease unless the expected benefits outweigh the risks. Some blood pressure medicines may work less well while taking Celebrex. Stop taking Celebrex and seek medical advice at the first sign of a skin rash or other allergic symptoms. Serious skin reactions and DRESS, a potentially serious reaction affecting several organs, require stopping treatment and medical assessment. Seek emergency help for a severe allergic reaction. Treatment should also be stopped if abnormal liver tests persist or worsen, or signs of liver disease develop. Use during pregnancy should be limited between about 20 and 30 weeks because of the risk of fetal kidney problems and low amniotic fluid. Avoid use from about 30 weeks onward because it can also prematurely close an important blood vessel in the unborn baby. NSAIDs can cause reversible fertility problems; discuss stopping Celebrex with your prescriber if you are having difficulty conceiving.
Interactions
Celebrex (celecoxib) can interact with the following medicines: • Anticoagulants such as warfarin, antiplatelet medicines such as aspirin, and SSRI or SNRI antidepressants can increase bleeding risk when taken with Celebrex. Your doctor should monitor for bleeding. • Aspirin used for pain relief is generally not recommended with Celebrex because it increases bleeding and stomach or intestinal side effects without providing greater benefit. Celebrex does not replace aspirin prescribed for heart protection. • Other NSAIDs and salicylates, including diflunisal and salsalate, are not recommended with Celebrex because they increase stomach and intestinal risks with little or no added benefit. • Corticosteroids can increase the risk of stomach or intestinal ulcers or bleeding. • ACE inhibitors, angiotensin receptor blockers (ARBs) and beta-blockers may become less effective at lowering blood pressure. ACE inhibitors and ARBs can also increase the risk of worsening kidney function when combined with Celebrex, particularly in older people, those who are dehydrated or those with kidney problems. Blood pressure and kidney monitoring may be needed. • Diuretics, including furosemide and thiazides, may work less effectively. Your doctor should check their effect and monitor kidney function. • Digoxin and lithium levels can rise. Your doctor should monitor digoxin blood levels and watch for signs of lithium toxicity. • Methotrexate may carry a greater risk of toxicity, including low blood cell counts and kidney problems. Cyclosporine may carry a greater risk of kidney damage. Monitoring is needed. • Pemetrexed may carry a greater risk of reduced blood cell production, kidney problems and stomach or intestinal toxicity. Your doctor should review the combination, particularly if you have kidney impairment. • Fluconazole and other medicines that block the liver enzyme CYP2C9 can increase celecoxib exposure and toxicity. Rifampin and other medicines that increase this enzyme’s activity may reduce celecoxib’s effectiveness. • Medicines processed by the liver enzyme CYP2D6, such as atomoxetine, may reach higher levels and cause more toxicity when taken with Celebrex. Your prescriber may need to adjust treatment.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
