If the QT interval prolongs to 500 msec or greater, reduce the dose, lengthen the dosing interval, or discontinue the drug.
Calculate creatinine clearance to determine appropriate dosing.

About Betapace
Overview
Betapace contains sotalol hydrochloride, an antiarrhythmic medicine used to treat life-threatening abnormal rhythms in the heart’s lower chambers. It is also used to maintain a normal heart rhythm in people with atrial fibrillation or atrial flutter, but should be avoided when these conditions cause minimal symptoms or are easily reversible. It works by blocking beta receptors and prolonging the heart’s electrical signals to help control its rhythm. Betapace has not been shown to improve survival in people with life-threatening abnormal rhythms in the heart’s lower chambers.
What it treats
• Life-threatening ventricular arrhythmias (abnormal heart rhythms arising in the heart’s lower chambers). Betapace has not been shown to improve survival in these patients. • Maintenance of normal heart rhythm in patients with atrial fibrillation or atrial flutter. Avoid use when symptoms are minimal or the rhythm problem is easily reversible.
Safety information
Side effects
Common side effects include tiredness, a slow heartbeat, shortness of breath, new or worsening abnormal heart rhythms, weakness and dizziness. Contact a doctor promptly for a slow heartbeat, shortness of breath, or new or worsening abnormal heart rhythms. Serious risks include QT prolongation (a change in the heart’s electrical activity), heart block, low blood pressure and worsening heart failure. Betapace may mask symptoms of low blood sugar and change blood sugar levels. Blood sugar should be monitored in people with diabetes. Electrolyte disturbances need to be corrected. Do not stop Betapace suddenly, as this can acutely worsen coronary artery disease.

Before taking this medicine
Do not take Betapace if you have: • A slow heart rate, sick sinus syndrome, or second- or third-degree heart block, unless you have a functioning pacemaker. • Congenital or acquired long QT syndrome. • Cardiogenic shock or decompensated heart failure. • A blood potassium level below 4 mEq/L. • Bronchial asthma or a related condition that causes the airways to narrow. • An allergy to sotalol. For treatment of atrial fibrillation or atrial flutter, do not take Betapace if your QT interval before treatment is longer than 450 milliseconds. Betapace can prolong the QT interval, slow the heart rate, cause heart block or low blood pressure, and worsen heart failure. These problems may require your prescriber to reduce or stop treatment. Any electrolyte disturbances must be corrected. Do not stop Betapace abruptly, as this can acutely worsen coronary artery disease. If you have diabetes, Betapace may mask signs of low blood sugar and alter blood glucose levels; monitoring is needed. Do not breastfeed while taking Betapace.
Interactions
Avoid taking Betapace with Class I or III antiarrhythmic medicines or other medicines that prolong the QT interval, a measure of the heart’s electrical activity. Tell your doctor if you take: • Digoxin or calcium channel blockers, which can increase the risk of a slow heart rate, low blood pressure or heart failure. • Insulin or other diabetes medicines, as your doctor may need to adjust them. • Antacids containing aluminum or magnesium, which can reduce the amount of sotalol available in the body.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
