Estrogen Plus Progestin Therapy
Cardiovascular Disorders and Probable Dementia
The Women's Health Initiative (WHI) estrogen plus progestin substudy reported increased risks of pulmonary embolism (PE), deep vein thrombosis (DVT), stroke, and myocardial infarction (MI) in postmenopausal women (50 to 79 years of age) during 5.6 years of treatment with daily oral conjugated estrogens (CE) [0.625 mg] combined with medroxyprogesterone acetate (MPA) [2.5 mg], relative to placebo.
The WHI Memory Study (WHIMS) estrogen plus progestin ancillary study of WHI reported an increased risk of developing probable dementia in postmenopausal women 65 years of age and older during 4 years of treatment with daily CE (0.625 mg) combined with MPA (2.5 mg), relative to placebo. It is unknown whether this finding applies to younger postmenopausal women.
Do not use estrogen plus progestogen therapy for the prevention of cardiovascular disease or dementia.
Breast Cancer
Prescribe estrogens with or without progestogens at the lowest effective doses and for the shortest duration consistent with treatment goals and risks for the individual woman.

About Angeliq
Overview
Angeliq is a prescription hormone medicine containing estradiol, an estrogen, and drospirenone, a progestin. It is used in women who have a uterus to treat moderate to severe menopausal hot flashes and symptoms of thinning vulvar and vaginal tissues. Estradiol acts on estrogen-responsive tissues, while drospirenone counters estrogen’s effects on the lining of the uterus. If vulvar and vaginal symptoms are the only reason for treatment, the label advises considering topical vaginal products first.
What it treats
- Moderate to severe hot flashes due to menopause in women with a uterus. - Moderate to severe symptoms of vulvar and vaginal atrophy due to menopause in women with a uterus. When treating only these symptoms, topical vaginal products should be considered first.
Safety information
Side effects
Common side effects include stomach or abdominal pain, vaginal bleeding, breast pain or discomfort, and headache. Serious risks include heart and blood vessel disorders, cancer, and gallbladder disease. Contact your doctor promptly if you develop or are diagnosed with any of these problems. Seek immediate medical advice for loss of vision, severely high blood calcium, severely high triglycerides (blood fats), or jaundice caused by impaired bile flow. The label advises stopping estrogen treatment if these occur. Angeliq must not be used in people with conditions that make high blood potassium more likely.

Before taking this medicine
Do not take Angeliq if you have unexplained abnormal genital bleeding; breast cancer or a history of it; an estrogen-dependent tumor; blood clots in the deep veins or lungs, or a history of these; or an arterial blood clot condition, such as stroke or heart attack, now or in the past. Do not take it if you have impaired kidney function, liver impairment or disease, adrenal insufficiency, or a known blood-clotting disorder such as protein C, protein S or antithrombin deficiency. It must not be used if you have a known allergy to Angeliq or any of its ingredients, including a previous severe allergic reaction or angioedema (swelling beneath the skin). Angeliq must not be used in people with conditions that make high blood potassium more likely. Estrogens increase the risk of gallbladder disease. Stop taking Angeliq and seek medical advice if you develop severely raised blood calcium, loss of vision, severely raised triglycerides (blood fats), or jaundice caused by impaired bile flow. If you take thyroid hormone replacement, your thyroid function needs monitoring.
Interactions
Tell your doctor about other medicines you take. Medicines that increase or block the activity of an enzyme called CYP3A4 may lower or raise estrogen levels in the blood. Taking Angeliq with other medicines that can raise potassium may increase blood potassium levels.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
