About Ambisome

Overview

AmBisome is a prescription liposomal amphotericin B medicine given by intravenous infusion. It treats suspected fungal infections in people with fever and low neutrophil counts, cryptococcal meningitis in people with HIV, and visceral leishmaniasis. It also treats certain Aspergillus, Candida or Cryptococcus infections when amphotericin B deoxycholate has failed or cannot be used because of kidney impairment or unacceptable toxicity. Its liposomal delivery system carries the antifungal antibiotic amphotericin B within tiny fat-based membranes.

What it treats

• Suspected fungal infection in patients with fever and low neutrophil counts. • Cryptococcal meningitis in patients with HIV. • Aspergillus, Candida or Cryptococcus infections when amphotericin B deoxycholate has not worked or cannot be used because of kidney impairment or unacceptable toxicity. • Visceral leishmaniasis. Relapse after initial parasite clearance was common in patients with weakened immune systems.

Safety information

Side effects

Commonly reported side effects with AmBisome include chills, nausea, vomiting, diarrhea, abdominal pain, headache, rash, itching, weakness and trouble sleeping. Low potassium, magnesium or calcium levels, raised blood sugar, swelling, and changes in kidney or liver blood tests were also reported. These events occurred during clinical studies and were not necessarily caused by AmBisome. Contact a doctor promptly if you develop chest pain, difficulty breathing, a fast heartbeat, confusion, blood in the urine or other unusual bleeding. Serious reported problems include reduced kidney function, abnormal liver tests, low blood oxygen, low blood pressure, bleeding in the digestive tract and sepsis (a serious response to infection). Low red blood cell, white blood cell and platelet counts were also reported. Electrolyte disturbances, including low potassium, can be serious even when there are no obvious symptoms.

When to call your doctor

• Chest pain or a racing heartbeat. • Shortness of breath. • New confusion. • Blood in your urine.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

AmBisome should not be used if you have a known allergy or have previously had an allergic reaction to amphotericin B or any other ingredient in the product, unless your treating doctor considers that the benefit of treatment outweighs the risk.

Interactions

No formal drug interaction studies have been conducted with AmBisome. However, medicines known to interact with amphotericin B may also interact with AmBisome. Tell your doctor if you receive: • Cancer medicines: these may increase the risk of kidney damage, narrowing of the airways and low blood pressure. • Corticosteroids or corticotropin (ACTH): these may worsen low potassium levels, which can affect heart function. Close monitoring of blood electrolytes and heart function is recommended. • Digitalis heart medicines: low potassium may increase their toxic effects. Blood potassium should be closely monitored. • Flucytosine: its toxic effects may increase when used with AmBisome. • Azole antifungals, such as ketoconazole, miconazole, clotrimazole or fluconazole: laboratory and animal studies suggest that some medicines in this group may cause fungi to become resistant to amphotericin B. Combined treatment requires caution, especially in people with weakened immune systems. • Other medicines that can damage the kidneys: combined use may increase kidney damage risk and requires intensive kidney function monitoring. • Skeletal muscle relaxants, such as tubocurarine: low potassium caused by amphotericin B may strengthen their muscle-relaxing effects. Blood potassium should be closely monitored. Also tell your doctor about any planned white blood cell transfusions. Acute lung toxicity has been reported when these were given at the same time as intravenous amphotericin B.

Storage

Store in the original carton below 25°C, protected from light, unless the pack states refrigeration. Do not freeze. Keep all medicines out of the sight and reach of children.
