About Aldactone

Overview

Aldactone is a prescription tablet containing spironolactone, a diuretic that blocks the hormone aldosterone. It treats advanced heart failure with reduced pumping ability and is used alongside other treatment for high blood pressure. It also treats fluid buildup in adults with cirrhosis or nephrotic syndrome when other measures are inadequate, and certain conditions that cause excess aldosterone production. It works by helping the kidneys remove more sodium and water while retaining potassium.

What it treats

- NYHA Class III–IV heart failure with reduced ejection fraction, to improve survival, manage fluid buildup and reduce hospitalizations for heart failure. - High blood pressure, as an add-on treatment. - Fluid buildup in adults with cirrhosis when fluid and salt restrictions are not enough, or with nephrotic syndrome when treatment of the underlying disease, fluid and salt restrictions, and other diuretics are not enough. - Primary hyperaldosteronism (excess aldosterone production), for short-term treatment before surgery or long-term treatment in patients with an aldosterone-producing adrenal adenoma who cannot have surgery, or with enlargement of both adrenal glands.

Safety information

Side effects

Breast enlargement in men (gynecomastia) is the most common side effect of Aldactone. Other reported side effects include nausea, vomiting, diarrhoea, stomach cramps or irritation, leg cramps, dizziness, headache, drowsiness and lack of energy. Breast or nipple pain, reduced sex drive, difficulty getting or maintaining an erection, irregular or absent periods, bleeding after menopause, hair loss and itching have also been reported. The frequency of these effects cannot always be reliably estimated. Contact a doctor if you experience serious side effects. Reported serious effects include: • High blood potassium, low blood sodium and other disturbances in blood salts. • Low blood pressure, reduced fluid volume and worsening kidney function, including kidney failure. • Stomach bleeding or ulcers. • Severe allergic reactions, including anaphylaxis, hives, fever and inflammation of blood vessels. • Severe skin reactions, including Stevens–Johnson syndrome, toxic epidermal necrolysis and DRESS, a drug reaction that can affect internal organs. • Low white blood cell or platelet counts. • Mental confusion or impaired coordination. Impaired brain function or coma can occur in patients with liver impairment, cirrhosis and fluid accumulation in the abdomen. • Liver injury, including a reported fatal case. Regular monitoring of potassium, other blood salts and kidney function is important because Aldactone can cause serious changes in these measures.

When to call your doctor

- Seek immediate medical attention for a severe skin rash or hives. - Get medical help immediately for severe confusion, loss of coordination, or extreme drowsiness. - Seek immediate medical attention for bleeding from the stomach.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Aldactone if you have high blood potassium (hyperkalemia), Addison’s disease, or are taking eplerenone. Aldactone can cause high blood potassium, low blood pressure, worsening kidney function, changes in electrolyte levels, uric acid or blood glucose, and breast enlargement (gynecomastia). Blood potassium should be checked within one week of starting treatment and regularly thereafter. Fluid balance, kidney function, electrolytes, uric acid and blood glucose should also be monitored periodically. Pregnancy requires caution: animal data suggest Aldactone may affect male sexual development in the embryo.

Interactions

Tell your doctor about medicines or supplements that raise blood potassium, as taking them with Aldactone can cause high potassium levels. Other interactions to discuss include: • Lithium: Aldactone increases the risk of lithium toxicity. • NSAIDs: These may reduce Aldactone’s effects on fluid and salt removal and blood pressure. • Digoxin: Aldactone can interfere with certain blood tests used to measure digoxin levels. • Cholestyramine: High potassium with excess acid in the blood has been reported with combined use. • Aspirin (acetylsalicylic acid): This may reduce Aldactone’s effectiveness. • Abiraterone: Combined use may increase prostate-specific antigen (PSA) levels. • Mitotane: Avoid taking it with Aldactone.

Dosing

Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
