About Aciphex
Overview
Aciphex delayed-release tablets contain rabeprazole sodium, a proton pump inhibitor (PPI). In adults, it is used to treat gastroesophageal reflux disease (GERD), heal and maintain healing of acid-related damage to the esophagus, heal duodenal ulcers, eradicate Helicobacter pylori to reduce duodenal ulcer recurrence, and treat conditions that cause excessive stomach acid, including Zollinger-Ellison syndrome. It is also used for short-term treatment of symptomatic GERD in adolescents aged 12 years and older. It reduces stomach acid by blocking the acid pumps in the stomach lining.
What it treats
- Healing and maintenance of healing of erosive or ulcerative gastroesophageal reflux disease (GERD) in adults. - Symptomatic GERD in adults, and short-term treatment of symptomatic GERD in adolescents aged 12 years and older. - Healing of duodenal ulcers in adults. - Eradication of Helicobacter pylori to reduce the risk of duodenal ulcer recurrence in adults. - Conditions causing excessive stomach acid production, including Zollinger-Ellison syndrome, in adults.
Safety information
Side effects
Common side effects in adults include pain, sore throat, gas, infection and constipation. In adolescents, common side effects include headache, diarrhea, nausea, vomiting and abdominal pain. Serious side effects that warrant contacting a doctor include kidney inflammation, diarrhea associated with Clostridium difficile infection, severe skin reactions or other allergic reactions, and new or worsening lupus affecting the skin or other parts of the body. Stop taking Aciphex and seek medical advice at the first signs of a severe skin reaction or allergic reaction. Long-term use may increase the risk of osteoporosis-related fractures of the hip, wrist or spine, vitamin B12 deficiency, and polyps in the stomach lining. Low magnesium and disturbances in mineral balance have been reported rarely with prolonged treatment. Discuss these risks with your doctor. Aciphex may increase warfarin’s effect on blood clotting, requiring monitoring. Taking it with methotrexate may increase methotrexate levels and the risk of toxicity. Tell your doctor if you take either medicine.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take Aciphex if you are allergic to rabeprazole, substituted benzimidazoles or any ingredient in the tablets, or if you take a medicine containing rilpivirine. The tablet strength covered by this label is not appropriate for children under 12 years. Tell your prescriber if you take warfarin or methotrexate. Warfarin requires monitoring of blood-clotting tests. Aciphex may increase methotrexate levels and toxicity; your prescriber may temporarily stop Aciphex during high-dose methotrexate treatment. If Aciphex is prescribed with clarithromycin and amoxicillin, the contraindications for both antibiotics also apply. Aciphex can cause kidney inflammation, serious allergic reactions, severe skin reactions, and new or worsening lupus. Stop treatment and seek medical assessment if a serious allergic or skin reaction is suspected. Suspected kidney inflammation or lupus also requires stopping treatment and medical evaluation. Other cautions include an increased risk of Clostridium difficile-associated diarrhea, osteoporosis-related fractures with long-term treatment, vitamin B12 deficiency with prolonged use, and rarely low magnesium or other mineral disturbances. Long-term use also increases the risk of growths in the stomach lining called fundic gland polyps, especially beyond one year. Improvement in symptoms does not rule out stomach cancer in adults; further testing or follow-up may still be needed.
Interactions
Aciphex (rabeprazole) can interact with the following medicines: • HIV medicines: Do not take Aciphex with rilpivirine-containing products. Nelfinavir should also be avoided. Aciphex may reduce the effects of some HIV medicines, including atazanavir, or increase side effects from others, such as saquinavir. • Warfarin: The combination can increase the risk of serious, potentially fatal bleeding. Blood-clotting tests need monitoring, and your doctor may need to adjust warfarin treatment. • Methotrexate, especially high-dose treatment: Aciphex may increase methotrexate levels and toxicity. Your doctor may consider temporarily stopping Aciphex. • Digoxin and tacrolimus: Aciphex may increase their levels. Blood-level monitoring and treatment adjustments may be needed. • Medicines that need stomach acid for absorption: Aciphex may reduce absorption of iron salts, erlotinib, dasatinib, nilotinib, mycophenolate mofetil, ketoconazole and itraconazole. Particular caution is needed in transplant patients taking mycophenolate mofetil. • Clarithromycin and amoxicillin: If these are prescribed with Aciphex, their interactions with your other medicines also need review. Some combinations involving clarithromycin can cause serious, potentially fatal heart rhythm problems. Aciphex can also affect test results. It may cause false-positive results in investigations for neuroendocrine tumors, secretin stimulation testing for gastrinoma, and urine screening for THC. Tell the clinician arranging these tests that you take Aciphex. Your doctor may need to pause Aciphex for at least 14 days before chromogranin A or secretin stimulation testing. A positive urine THC screening result may need confirmation using another method.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
